How to Enroll

You have the power to control your future – your health, your money, your life. Selecting benefits is easy! Enroll one of two ways:

1. Enroll online through ADP! Simply log in, start enrollment, and follow the prompts.

Please be prepared to provide a date of birth and social security number for each dependent you are adding to your plan(s). You will also need the date of birth and social security numbers for each beneficiary.

Medical coverage is important for you and your family. You can choose between two (2) different Anthem medical plan options. Both Medical plans are considered Affordable and meet ACA standards for Essential Coverage.

Airstream Onsite Health Center
Associates can use the clinic at no cost (family members will be able to use the clinic at a later date). Services such as sending out blood work and referrals to specialists will still be billed through insurance.

Live Health Online
LiveHealth Online lets you have a video visit with a board-certified doctor using your smartphone, tablet or computer with a webcam. NO appointments, driving, or waiting at an urgent care. Doctors are available 24/7 and 365 days a year to assess your condition and if needed, they can send a prescription to your local pharmacy. Your cost will not exceed $59.

Your Dental plan is provided through Delta Dental of Indiana. Find a dentist at deltadentalin.com. Your Plan pays $1,500 per person total per Benefit Year. Your dentist will work directly with Delta Dental to determine your cost and will review these costs with you prior to your treatment.

You are responsible for a $50 Deductible per person total per Benefit Year limited to a maximum Deductible of $150 per family per Benefit Year. The Deductible does not apply to oral exams, preventive services, X-rays, brush biopsy, sealants, and orthodontic services.

If you continue to work full-time, you may remain on the company medical plan if you meet the eligibility requirements. However, you may also be eligible for Medicare A & B, a Medicare Supplement and Medicare D. Please read the summary below and explore your options to determine what is best in your situation.

Working Beyond Age 65: If you are purchasing medical insurance through your employer, a Medicare plan could help you save money on your health care expenses. It may make sense for you to sign up for Medicare in addition to OR instead of the coverage you have today. If you enroll in Medicare and remain on the company health plan, be sure to check the coordination rules to determine which coverage is primary.

Medicare Options: Many people who choose to work past age 65 enroll in Part A (Hospital Insurance) because there is no monthly premium. You may choose to enroll in Medicare Part B, a Medicare Supplement, and/or Medicare Part D (these options will be subject to a monthly premium cost)

  • Medicare Part B – Physician Insurance
  • Medicare Part D – Drug Coverage
  • Supplemental Coverage – This can include Medigap coverages, employer plans or Medicaid.

It is recommended that you explore all options to determine what is best for you. You may also shop for, and change plans each year based on your specific needs.

Understanding Your Options: Associates who choose to remain on the group health plan can sign up for premium-free Part A (if eligible) during or after their Initial Enrollment Period begins. You can only sign up for Part B (or Part A if you have to buy it) during certain enrollment periods as dictated by Medicare. For additional information on Medicare enrollment opportunities visit www.medicare.gov or reach out to your local SHIP office (see Medicare Resources for contact information).

Making Changes to Your Medicare Plans: Health care needs can change from year to year. Be sure to review your needs annually (upcoming surgeries, current prescription drugs, new wellness goals) so you can find a plan to best meet them.

Medicare Open Enrollment Period: You can enroll in or change your plan once a year during the Open Enrollment Period (OEP) even if you do not have a qualifying event. The OEP is a seven-week period from October 15 through December 7.

Retiring At or After Age 65: Whether you retire or decide to work part-time, once you turn age 65 you will be eligible for Medicare (Parts A and B) and other Medicare Supplement Plans. If you don’t have employer sponsored coverage, you should consider enrolling during your Initial Enrollment Period. You can enroll any time within the 3 months before your 65th birthday month, your birthday month, or 3 months after.

You are automatically enrolled on the first day of the month after 90 days at a 6% contribution. Choose a savings approach that suits you today – and adjust it any time to fit your changing needs. You can update your contributions online at Netbenefits.com/easy.

Establish your beneficiaries
It’s important to name and regularly review and update beneficiaries for your 401(k)
retirement plan benefits to prevent benefits being be paid according to Plan rules, which might be different from the designation you would choose. Please take a few moments today to name your beneficiaries online to ensure that your benefits will be distributed according to your wishes.

To navigate to your beneficiary designations online:

  1. Log into your account at www.netbenefits.com.
  2. Click the Your Profile link.
  3. Select Beneficiaries and follow the online instructions.

Once you have completed your beneficiary designations, you will be able to view them on NetBenefits. Please be sure to review your choices regularly and update them after certain life events, such as a marriage, divorce, birth of a child, or a
death in the family.

Wellness Benefit ▼

Airstream has partnered with our Onsite Clinic to focus on our Associate’s health and wellness.

  • Associate’s will receive a $200 bonus if they complete their annual physical between January 1st 2024 and June 1st 2024.
  • Spouses can also earn a $100 bonus if they too complete their annual physical between January 1st and June 1st 2024.

Physicals and biometric screenings can be scheduled at the Clinic or they can be completed by the Associate’s Primary Care Physician. If an Associate chooses to have their physical with their PCP, the PCP will complete a form and the Associate will need to provide it to the Clinic.

Life and accident insurance gives you financial protection by paying a benefit if you die or are seriously injured in an accident.

Basic Life and AD&D Insurance
As an important part of your personal financial planning, Airstream provides you
with life and accidental death and dismemberment (AD&D) insurance coverage through Mutual of Omaha at no cost to you. AD&D insurance is designed to
pay a benefit if you have an accident that results in a covered loss, or death.

Voluntary Life Insurance
Airstream offers you the opportunity to purchase additional life insurance for yourself and your covered dependents through payroll deduction. Premiums are based on the amount of life insurance elected and the age of the insured. Note: Associate must be enrolled in voluntary life in order to elect spouse or dependent life.

Voluntary Life Insurance Features

Associate Coverage

Purchase in $10,000 increments up
to the greater of 5x salary or
$300,000.

Statement of Health/EOI required
for amounts over $200,000 or if you
enroll anytime after you were first
eligible as a new hire or after this

Universal Life Insurance ▼

Airstream offers you an opportunity to increase your life insurance coverage with a whole life insurance offering through Trustmark.

What is universal life/life events?
Trustmark Universal Life/LifeEvents® is permanent life insurance. It provides a death benefit to your beneficiaries if you pass away, but also builds cash value and features living benefits for long-term care services. Your price won’t increase due to age, and your policy builds cash value over time.

What is long term care?
Long Term/Convalescent Care Benefit (LTC) pays 4% of your death benefit for up to 25 months for home healthcare, assisted living, nursing home care and adult day care. There is a 90-day elimination period before benefit can be paid. To receive benefits you must meet Conditions of Eligibility for Benefits. The LTC benefit does not reduce at age 70. Benefit Restoration restores the death benefit that is reduced to pay for LTC, so your family receives the full death benefit amount when they need it most.

In the event of a non-work related accident or any non-work related debilitating event, a portion of your salary will be continued for a period of time.

Short-term disability (STD)
Your ability to earn an income may be your most important asset. Short-term Disability insurance pays you cash benefits when you are sick or hurt (non-work related) and unable to work.
