To ensure you understand your benefit options, each benefit-eligible employee will have the opportunity to speak with a benefit counselor over-the-phone.

  • Receive help choosing the best plans for your needs
  • Enroll in Dental, Vision, Term Life, Short-Term Disability, Long-Term Disability, Critical Illness, Accident, and/or Hospital Indemnity Insurance.

With Voluntary Term Life Insurance, you can enhance financial security for yourself and your family. Choose coverage that fits your needs, with options ranging from $25,000 to $500,000 for yourself and additional coverage for your spouse and dependents. You’ll also have access to estate guidance services and enhanced accidental death benefits. Coverage is guaranteed for up to $200,000 for you and $50,000 for your spouse without medical underwriting during this enrollment period. There are times when many disabilities can be caused by illness, including common conditions like heart disease and arthritis. However, many disabilities aren't covered by workers' compensation.

Who is it for?
If you rely on your income to pay for everyday expenses, then you should probably consider disability insurance. It helps ensure that you’ll receive a partial income if you’re injured or too sick to work.

What does it cover?
Many disability insurance plans pay out a portion or percentage of your income if you’re diagnosed with a serious illness or experience an injury that prevents you from doing your job.

Why should I consider it?
Accidents happen, and you can’t always anticipate if or when you’ll become sick or injured. That’s why it’s important to have a disability policy that helps you pay your bills in the event of being unable to collect your normal paycheck.

  • Benefits Begin: 15th day for accident or sickness.
  • Benefit Duration: 13 weeks.
  • Disability Definition: Based on the ability to perform your own job.
  • Weekly Benefit:
    - Plan 1: 60% of earnings, up to $462.
    - Plan 2: 60% of earnings, up to $1,000.
  • Elimination Period Interruption: No set limit on the number of days.
  • Return to Work: Zero-day residual requirement.
  • Partial Disability: Benefits are indexed at 80%, with calculation based on direct reduction or proportional loss.
  • Minimum Weekly Benefit: $25 flat.
  • Pre-Existing Condition Limitations: 3/12 rule with a 2-week limitation.
  • Claims Process: TeleGuard included for phone-based claims.
  • Coverage Type: Non-occupational.
  • Worksite Modification: Up to $2,500 available.
  • Recurrent Disability: 2-week period for recurring disabilities.

  • Survivor Benefit: 3 months accelerated payment for beneficiaries.

Accident insurance is an extra layer of protection that gives you a cash payment to help cover out-of-pocket expenses when you suffer an unexpected, qualifying accident.

Who is it for?
Nobody can predict when an accident might happen. That’s why accident insurance is an important add-on policy for people who want to supplement the health and disability insurance coverage they already have individually or through an employer.

What does it cover?
Accident Insurance pays you lump sum of benefits after you suffer an accident. This could be more than 40 different circumstances, including: emergency treatment, ambulance, burns, dislocations, fractures, hospital confinement, and surgery.

Why should I consider it?
Health coverage may become more expensive, with higher co-pays, premiums, and deductibles. Accident insurance can be a simple, affordable way to help supplement and cover additional expenses your health and disability insurance may not cover, including x-rays, ambulance services, deductibles, and even things like rent or groceries. Plus, accident insurance is portable and payments are made directly to you.

  • Wellness Benefit: Provides a $50 per year benefit for completing certain routine wellness screenings or procedures (refer to Plan Highlights section for example procedures)
  • Child(ren) Age Limits: Birth to 26 years subject to state limitations

  • Child Organized Sport: 25% increase to child benefits 
  • Chiropractic Visits: $25 per visit up to 6 visits

  • Emergency Room Treatment: $200
  • Eye Injury: $300
  • Hospital Admission: $1,250
  • Hospital Confinement: $300/day - up to 1 year
  • Hospital ICU Admission: $2,500

Why should I consider it?
Health coverage is becoming more expensive, with higher co-pays, premiums, and deductibles. Critical illness insurance is an affordable way to supplement and pay for additional expenses that your health insurance doesn’t cover. Our policies typically provide payments for the first and second time you’re diagnosed with a covered illness. Plus, critical illness insurance is portable and payments are made directly to you.

Coverage includes diagnoses for the following conditions:

  • Heart conditions, such as heart attack, sudden cardiac arrest resulting in death, or mitral or aortic valve disease
  • Cancer, such as invasive cancer, noninvasive cancer and skin cancer
  • Kidney failure or major organ transplant
  • Stroke or arterial/vascular disease
  • COVID-19 is a qualifying condition

Being hospitalized for illness or injury can happen to anyone, at any time. While medical insurance may cover hospital bills, it may not cover all the costs associated with a hospital stay. That’s where hospital indemnity coverage can help.

Who is it for?
Hospital indemnity insurance is for people who need help covering the costs associated with a hospital stay if they suddenly become sick or injured.

What does it cover?
If you are admitted to a hospital for a covered sickness or injury, you’ll receive payments that can be used to cover all sorts of costs, including:

  • Deductibles and co-pays.
  • Travel to and from the hospital for treatment.
  • Childcare service assistance while recovering.

Why should I consider it?
Health coverage is becoming more expensive, with higher co-pays, premiums, and deductibles. Hospital indemnity insurance can help pay for out-of-pocket costs associated with being hospitalized, giving you more of a financial safety net for unplanned expenses brought on by a hospital stay.

Plus, hospital indemnity insurance is portable and payments are made directly to you – even if you didn’t incur any out-of-pocket expenses.

You will receive these benefits if you meet the conditions listed in the policy.

Benefits:

  • Hospital/ICU Admission: $1,500 per admission to a max of 1 admission per year, per insured
  • Hospital/ICU Confinement: $100 per day to a max of 30 days per year, per insured
  • Emergency Room/Urgent Care Facility Treatment: $150 /$75 per day to a max of 1 day per year, per insured
  • Inpatient Surgical Procedure: $500/$1,000 per day of surgery to a max of 1 day per year, per insured
  • Rehabilitation Unit Confinement: $250 per day to a max of 5 days per year, per insured 
  • Doctor's Office Visit: $50 per day to a max of 3 days per insured, per year, max of 6 days per year, per covered family 
  • Diagnostic Tests: $250 per day to a max of 1 day per year, per insured
  • Dependent Age Limits: Child Birth to 26 years (26 if full time student)
  • Treatments Covered: Sickness and injury
  • Treatment of Normal Pregnancy: Hospital Admission benefits are not payable for birth within first 9 months of coverage. See Plan Limitations & Exclusions section for details.

Your new hire enrollment period is the only opportunity to enroll in benefits until the next open enrollment period unless you experience a qualifying life event and request a change to your benefits within 30 days of the life event.
