You may enroll in one of two medical plan options:

  • Gold
  • Silver

Each plan includes access to the BlueChoice PPO network. You may visit any doctor or specialist of your choice without a referral. When you use an in-network provider, you will receive benefits at a higher level resulting in less out-of-pocket expenses.

How to Find a Network Provider
Finding an in-network doctor is easier than ever. Most importantly, you are in control when searching on bcbstx.com. After logging into Blue Access for Members, your search results are personalized according to
your health plan, so you get the information you need to make the right decisions for you and your family.

Flexible Spending Account (FSA) ▼

Administered by TaxSaver Plan

Flexible Spending Accounts (FSA) provide you with an important tax advantage that can help you pay for health care and dependent care expenses on a pre-tax basis. By estimating your family’s health care costs for the next year, you can lower your taxable income and save money. There are two types of Flexible Spending Accounts:

  • The Health Care FSA
  • The Dependent Care FSA

You may participate in one or both accounts. You may deposit up to $3,200 per year in a Health Care FSA and/or up to $5,000 per year in a Dependent Care FSA.

Please note that these accounts are separate — you may choose to participate in one, both, or neither. You cannot use money from the Health Care FSA to cover expenses eligible under the Dependent Care FSA or vice versa.

You must actively re-enroll in either FSA plan each year. You are not automatically re-enrolled.

USE IT OR LOSE IT
Submit claims up to March 31 of the following year for expenses from January 1 to December 31. If you do not spend all the money in this FSA by March 31, per IRS regulations, unused dollars will be forfeited for pretax contributions.

Dental ▼

Keep a smile on your face and more money in your pocket with our dental plan. All employees have the option to enroll in the Dental PPO plan. As a member of the Dental PPO plan, you may use dentists outside of the Dental PPO plan network.

American Leather and Brookline employees also can enroll in the DHMO plan. If you are enrolled in the DHMO plan, you must use in-network providers only in order for benefits to be paid by this plan. The DHMO provides set copay amounts for services received.

Disability ▼

Administered by Lincoln Financial Group

Short-Term Disability (STD) Insurance
American Leather, Brookline Furniture, and Lee Industries provide you with Short-Term Disability coverage through Lincoln Financial Group. The benefit amount is based on 60% of your income, up to $500 per week. This amount may be offset by other income benefits, including any local or state disability. Benefits begin at time of disability for accidents and after seven days for illnesses. This coverage can provide benefits for up to a maximum of 13 weeks if continuously disabled.

Long-Term Disability (LTD) Insurance
As an added protection to ensure your financial stability, American Leather, Brookline Furniture, and Lee Industries also provide you with Long-Term Disability coverage through Lincoln Financial Group. If your disability extends beyond 90 days, this coverage can replace 60% of your earnings, up to a maximum of $6,000 per month. Your benefits may continue to be paid until you reach normal retirement age as long as you meet the definition of disability.

401(k) Plan ▼

A 401(k) is a type of retirement plan offered through your workplace. Here are a few reasons why you should participate:

  • Beyond providing a way to invest for retirement, 401(k)s may offer huge tax advantages. For example, in some plans your contribution comes out of your paycheck before income taxes are deducted, which means your tax bill will be lower. You could also save on taxes when you withdraw the money because you may be in a lower tax bracket in retirement.

Life Insurance with Long Term Care ▼

LifeTime Benefit Term Insurance with Long Term Care (LTC) provides lifelong protection with flexible premiums. It is one policy with two benefits - life insurance coverage for your family and long term care benefits if you need them.

As Life insurance, this plan protects your family with money that can be used any way they choose. It is most often used to pay for mortgage or rent, education for children and grandchildren, retirement, family debt, and final expenses.

Program Highlights

  • Coverage is available for yourself, your spouse, and your child(ren).
  • Flexible permanent insurance designed to last a lifetime.
  • Benefits for long term care services such as home health or nursing home care - you can access 4% of the selected life benefit per month if you need LTC.
  • Death Benefit is guaranteed 100% when it is needed most – during the working years when a family is relying on income. While the policy is in force, the death benefit is guaranteed for the longer of 25 years or through age 70.
  • Even after age 70, when income is less relied upon, the death benefit is guaranteed to never be less than 50% of the original death benefit.

During this enrollment period only, you have a special opportunity to elect coverage, up to $100,000 for yourself and up to $25,000 for your child(ren), without answering medical questions! You can elect up to $75,000 for your spouse after answering a few medical questions.

When you suffer an accident (on and off-the-job) such as a burn or broken bone, Accident Insurance provides a lump-sum cash benefit based on your injury(s) and the treatment you receive. These benefits are paid on top of what your health insurance covers, and can be used at your own discretion. We are proud to offer an enhanced plan with increased benefit payouts on fractures, dislocations, ER visits, and hospital stays.

And every year that you complete a qualified health screening, you are eligible for a Wellness Benefit of $50 or $75 per insured per year.

Plus, the policy will pay you a $50 wellness benefit per insured, once per year when you and/or your covered dependents complete a qualified health assessment. New this year: The enhanced wellness benefit now covers 12 additional qualified health assessments including Biometric screenings and no benefit waiting period.

Also new this year, this plan now covers 3 additional critical illnesses for adults, 7 additional critical illness for children, an increased benefit payout for Breast Cancer, and unlimited benefit payouts.

Hospital Indemnity Insurance can reduce the financial and emotional stress of a hospital stay by providing a lump-sum cash benefit directly to you that can be used however you need, whether that’s for coinsurance or childcare.

Life and AD&D ▼

Basic Life and AD&D Insurance
Basic Life Insurance helps ease your loved ones’ financial burden. Your designated beneficiary will receive a benefit
if you pass away from a covered accident or illness. In addition, Accidental Death and Dismemberment (AD&D) provides a benefit to your beneficiary if you pass on or become dismembered due to a specifically covered accident. Always make sure your beneficiaries are updated.

Eligible employees are covered in the amount of either $30,000 or one and one-half times your basic annual earnings, up to a maximum of $150,000.

Voluntary Life and AD&D Insurance
You may also choose to purchase Voluntary Life Insurance coverage in addition to the company-paid benefit. You pay the total cost of this benefit through convenient payroll deduction.

Wellness Program ▼

All medically enrolled employees and spouses will have the opportunity to participate in an annual physical to earn a Wellness Incentive towards your Medical Premium in 2025.

Incentive Instructions
