Medical ▼

ATI offers three medical plans. In addition to the Standard PPO Plan, we offer 2 High Deductible Health Plans (HDHP).

Choice is important when selecting the right medical plan. Take the time to learn about your options to ensure you select the right plan for you and your family. All plans have access to the BCBSIL PPO Network. Each plan pays benefits for in-network and out-of network services.

  • High Deductible Health Plan (HDHP 5000): $5,000 single/$9,100 family
  • High Deductible Health Plan (HDHP 3000): $3,000 single/$6,000 family
  • Standard PPO Plan

MDLive Telemedicine

MDLIVE is available for all ATI medical plan participants regardless of the plan you enroll in. The MDLIVE benefit gives you access to physicians on-line, via phone call or mobile app and allows you to request prescriptions during or after normal business hours - including nights, weekends and holidays. You can also use MDLIVE for non-emergency medical care and for when you need medical care while traveling.

They can treat conditions that are easily diagnosed such as ear/sinus infections, headache, respiratory conditions, sore throats, UTI, cold and flu, and Behavioral Health Visits.

For the HDHP, the cost is less than a regular visit and for the Standard PPO plan the copay is the same as an office visit. Plus, it allows you to connect with a doctor in the comfort of your home or office without needing a pre-scheduled appointment. Most visits take around 10-15 minutes and if needed, a doctor can write a prescription for you (some states have limitations) that you can pick up at your local pharmacy.

Prescription Program
Every year drugs are moved on and off the Performance Drug List, while others may have a generic become available. We encourage you to check the Performance Drug List every year to avoid surprises at the pharmacy. For additional details on the prescription drug program, please refer to the Medical Plan Comparison chart on page 5 of the benefit guide.

As an added benefit the 90DayMyWay program allows you to receive a 90-day supply of maintenance drugs from one of the extended supply pharmacies or through the mail order program with AllianceRx Walgreens Prime. For a listing of maintenance drugs, you may access the website at www.bcbsil.com or contact customer service at the toll-free number on the back of your identification card.

Specialty Pharmacy
Specialty medicines are generally higher-cost medicines that require extra support to manage and administer. These medicines are used to treat chronic (long-term) and complex conditions.

This plan offers you the flexibility to visit any licensed dentist, but you will stretch your benefit dollars by visiting a dentist in MetLife's dental PPO network. MetLife's In-Network dentists have agreed to accept negotiated fees for covered services, which are typically 30-45% less than the average fees charged by Out-of-Network dentists in the same community.

Preventive services, such as cleanings and regular check-ups, are covered at 100% with no deductible both In-Network and Out-of- Network.

Accident ▼

When you suffer an on- or off-the-job accident such as a burn or broken bone, Accident Insurance through Reliance Standard provides a lump-sum cash benefit based on your injury(s) and the treatment you receive. These benefits are paid on top of what your health insurance covers and can be used at your discretion. This policy also pays an additional 25% on each benefit for injuries sustained by a child under age 18 in a school-sanctioned sport or a competitive sport requiring registration. And every year that you and a covered spouse complete a qualified health screening, you are eligible for a $75 HealthScreening Benefit.

Critical Illness ▼

No one can be completely prepared when a critical illness strikes, but if you or a loved one is diagnosed with a covered condition such as cancer, stroke, or heart attack, Critical Illness Insurance through Reliance Standard provides a lump-sum cash benefit to help pay for out-of-pocket medical expenses or any other bills that need attention, including rent or groceries. It can help provide financial protection so you can focus on what’s really important — getting better. You also have the option of picking your level of coverage so you can make sure you have the right protection for your family. And every year that you and a covered spouse complete a qualified health screening, you are eligible for a $50 Health Screening Benefit.

Pet Insurance will help cover your expenses if your pet is sick or injured and needs prescriptions, diagnostic tests, hospitalization or surgeries. ATI_HSA

Health Savings Account (HSA) ▼

ATI partners with HealthEquity to administer our employee’s Health Savings Accounts (HSA). 

Additionally, HSA accounts with HealthEquity will be free to ATI employees and there will no longer by monthly maintenance fees. Whether you are newly enrolling in one of the HDHP Plans or have historically had an HSA through us, ATI will work with HealthEquity to automatically open an account on your behalf and any new payroll contributions in 2024 will be sent to your Health Equity HSA Account. You will receive a new debit card in the mail along with instructions on accessing your account.

The HSA is like a bank account you use to pay for eligible health care expenses - such as office visits, prescription drugs and lab tests. The money you put into your HSA will reduce your taxable income for the year. Unused funds rollover at the end of the year and you take the money with you if you change health plans, change your employer or retire.

Team members who elect the HDHP 5000 Plan are eligible to open a Health Savings Account that will allow you to save pre-tax dollars for future qualified medical expenses. ATI will help fund your HSA by providing $300 for single coverage and $600 for anybody covering dependents.

Some key advantages to a HSA:

  • Tax Savings: Your contributions to the HSA are made with pre-tax dollars, so you’ll pay less in income taxes.
  • Control: You can use the HSA to pay for any qualified medical expenses, as defined by the IRS.
  • Savings and Investments: Unused HSA dollars roll over year to year. At age 65, you will have the ability to use your HSA funds for any purpose on a taxable basis.
  • Portability: The account is yours; you can take your HSA with you if employment changes.
  • Contributions and Investment Earnings: They are tax free, as are disbursements from the account to pay for qualified expenses.
  • You will receive a debit payment card, issued by HealthEquity.
  • In order for a dependent under age 26 to use HSA money, they must qualify as your TAC Code Dependent. Additional rules apply if your spouse also has an HSA. Please contact HealthEquity for specific information at 877.713.7682 or www.myhealthequity.com.

LifeTime Benefit Term Insurance with Long Term Care ▼

As life insurance, LifeTime Benefit Term Insurance with Long Term Care through Chubb protects your family with a death benefit that can be used any way they choose, most often for the mortgage or rent, education for children, debt, and final expenses. This policy includes living benefits that provide financial support and resources to cover the cost of long term care you might need as a result of an accident, illness, or aging. Coverage is permanent and flexible.

During your new hire enrollment period only, you can elect up to $100,000 of coverage for yourself without answering any medical questions.

Life Insurance ▼

Basic Life Insurance

Basic Life is an ATI paid benefit which provides a $50,000 life insurance policy for employees only. You will automatically be enrolled with this benefit at no cost to you.

Supplemental Life Insurance

ATI provides you the opportunity to purchase additional supplemental life insurance coverage for yourself, your spouse and your dependent children. Domestic partners with no affidavit on file are not eligible for spouse supplemental life insurance.

Employees: $10,000 increments to a maximum of $500,000.

Employee Guaranteed Issue for new hires, newly eligible employees and enrollments/increases due to a qualified life event: $500,000 (may be subject to providing proof of good health).

During your new hire enrollment period only, you can elect coverage without answering any medical questions.

Spouse: $5,000 increments to a maximum of $100,000

Spouse Guaranteed Issue for new hires, newly eligible employees and enrollments/increases due to a qualified life event: $25,000 (may be subject to providing proof of good health).

During your new hire enrollment period only, you can elect spousal coverage without answering any medical questions.

Child: $10,000 (Live birth to age 26, no proof of good health needed)

If you would like to enroll in additional life insurance, please see the online application and Evidence of Insurability instructions available on Dayforce.

Important! You must elect Employee Supplemental Life Insurance in order to elect Spouse or Child Supplemental Life Insurance.

Evidence of Insurability (EOI) is required for any amount of Employee or Spouse Supplemental Life coverage if you are enrolling beyond 31 days from the date you first become eligible for coverage.

Coverage requiring approval of good health will be effective the first of the month following approval.

Disability ▼

Short-Term Disability

Administered by ATI Physical Therapy

Short-Term Disability insurance provides you with income protection in the event of an injury, hospital admission or illness. You are eligible for STD coverage on the first day of the month following your date of hire, and you will be automatically enrolled with this benefit at no cost to you.

STD: Benefits begin on the eighth consecutive day of total disability and will not extend beyond 12 weeks. The maximum benefit is an amount equal to 80% of your covered earnings for the first five weeks, and 50% for the next seven weeks. STD Benefits are paid through normal ATI payroll.

Note: You are eligible for STD benefits for your pregnancy as you would be with any other disability. Remember to enroll in the BCBSIL Women’s and Family Health Pregnancy program to give your baby a healthy start.

Long-Term Disability

Administered by Reliance Standard

Long-Term Disability insurance continues a portion of your pay in the event that you are disabled beyond approved short-term disability. LTD benefits begin on the 90th consecutive day of total disability and will not extend beyond Social Security retirement age or the duration of benefits listed in the plan’s certificate. The monthly benefit is an amount equal to 60% of your covered base earnings, up to a maximum benefit of $5,000 per month.

Note: You are eligible for LTD benefits on the first day of the month, following 1 year of continuous service.

The ATI Holdings, LLC 401(k) Profit Sharing Plan offers you an easy way to save for retirement.

To be eligible to join and contribute to the 401(k) Plan, an employee must be 21 years or older*.

Features of our plan include:

  • Deferrals from 1% to 80% of your salary
  • Maximum employee contribution of $23,000 per plan year
  • Additional employee catch-up contribution of $7,500 if you are 50 years or older
  • Match begins after one year of service and 1,000 hours & starts the first of the following quarter

Global Core ▼

Like your passport, always carry your BCBSIL ID card with you when you travel or live abroad. Through the BCBS Global Core program, you have access to doctors, hospitals and other health services in nearly 200 countries and territories around the world.

How it works

To take advantage of the program, review the information below:

  • Before you leave home, contact BCBSIL for coverage details. Your coverage outside the United States may be different.
  • Always carry your BCBSIL ID card.
  • In an emergency, go directly to the nearest hospital.
  • The Global Core Service Center is available 24 hours a day, seven days a week toll-free at 800.810.2583 or by calling collect at 804.673.1177. See below for which situations to call the Service Center.
    • You need to locate a doctor or hospital or need medical services. They will arrange a physician appointment or hospital stay.
    • You need inpatient care. In this situation, you should also call BCBSIL for pre-certification. You can find the number on the back of your ID card. This is a separate number from the Global Core Service Center.

Payment and Claims Information

For participating hospitals, you shouldn’t need to pay up front, except for the usual out-of-pocket expenses. The hospital will file your claim for you as long as you arranged your service with the Global Core Service Center beforehand.

For doctors and non-participating hospitals, you will need to pay up front for services. Then you can complete an International Claim Form and send it with the bill(s) to the address on the form. The form is available from BCBSIL, the Service Center or bcbsglobalcore.com.

Whether you are pregnant or planning to get pregnant, you should prepare as much as you can. Blue Cross and Blue Shield of Illinois (BCBSIL) has tools to help you – at no extra cost to you.

Ovia HealthTM apps are for tracking your cycle, pregnancy and baby’s growth. The apps are available in English and Spanish, and provide videos, tips, coaching and more.

  • Ovia Fertility: Track your cycle and predict when you are more likely to get pregnant.
  • Ovia Pregnancy: Monitor your pregnancy and baby’s growth week by week leading up to your baby’s due date.
  • Ovia Parenting: Keep up with your child’s growth and milestones from birth through three years old.

Well onTarget® has self-guided courses about pregnancy that you can take online, covering topics such as healthy foods, body changes and labor.

Plus if your pregnancy is high-risk, BCBSIL will provide support from maternity specialists to help you care for yourself and your baby. Having a baby changes everything, so use these tools to help get you ready.

ATI_Commuter

Commuter Transit & Parking Accounts ▼

Administered by HealthEquity

A Commuter account is a pre-tax benefit that can save you up to a third of what you pay for parking and public transit as part of your daily commute, which includes train, subway, bus, ferry and eligible vanpool.

You can contribute up to $300 for the transit benefit and up to $300 for the parking benefit. You are able to change the election for each on a monthly basis. Once the funds are moved into the account, you can start using them for qualified commuting expenses. The money rolls over month to month and year to year as long as you are an ATI employee.
