How to Enroll

To make your benefit elections and complete your enrollment, you must use the online self-service portal. Click here or the button below to log on and enroll in your benefits for this plan year.

Self-service enrollment requires you to have the following information on hand so that you can log into the system:

  • USERNAME: First initial of your first name + first four letters of your last name + last four digits of your SSN # (all lowercase and no commas)
    Example: John Smith (SSN #: 123-45-6789) = jsmit6789
  • PASSWORD: Full date of birth (MMDDYYYY) Example: November 26, 1980 = 11261980
  • COMPANY: Elite Staffing

The vision plan is offered through VSP. This plan offers one eye examination per year, new lenses or contact lenses each year, and frames every two years.

You can choose from independent doctors and retail providers to find the one that best fits your needs and schedule. No matter which one you choose, the plan is designed to be easy to use and to save you money.

  • Click "Find a Doctor"
  • You can choose Search by Location, Office, or Doctor
  • Enter the information and click "Search"

Accident Insurance

What is Accident Insurance? ▼

When you suffer an accident (on and off-the-job) such as a burn or broken bone, Accident Insurance through Allstate provides a lump-sum cash benefit based on your injury(s) and the treatment you receive. These benefits are paid on top of what your health insurance covers, and can be used at your own discretion.

  • Coverage is available for you, your spouse, and dependent children
  • Coverage may be continued (with certain stipulations). That means you can take it with you if you change jobs or retire.

How to File a Claim ▼

Log into the MyBenefits website at mybenefits.allstate.com and follow the instructions below:

  • From the Claim Center, click File a Claim.
  • Verify or update your address and your claim payment method, then click the file a claim button under the appropriate policy.
  • Enter your Claim Details, including whether this is a new or ongoing claim.
  • Scroll down and enter at least one Treatment Type - NOTE: You can enter more than one Treatment Type for the claim
  • Scroll down to the Supporting Documentation section and drag your supporting documents into the Secure File Upload box, or click in the box to browse your computer for your documents. - NOTE: Supporting documents should show the condition/diagnosis, treatment, and any services received as well as the claimant’s name, provider name and dates of service.
  • Click the upload button and your supporting documentation will show in the Uploaded Files box. Click continue.
  • Review your Claim Information on the next page, then scroll to the bottom and click apply e-signature.
  • A confirmation page shows that your claim has been signed and submitted. You can print this page using the print button on the right.
  • You can check the Claim Center to see the status of your claim or upload additional claim information. - NOTE: Some claims that are submitted after 9 p.m. ET may not appear in the Claim Center until the following business day.

Critical Illness Insurance

What is Critical Illness Insurance? ▼

No one can be completely prepared for when critical illness strikes, but if you or a loved one is diagnosed with a covered condition such as cancer, stroke, or heart attack, Critical Illness Insurance through Allstate provides a lump-sum cash benefit to help pay for out-of-pocket medical expenses or any other bills that need attention, including rent or groceries.

  • Kidney Failure (End-Stage Renal Failure)
  • Major Organ Transplant

  • Coronary Artery Bypass Surgery
  • Non-Invasive Cancer
  • Skin Cancer

Plan Features:

  • Benefits are paid directly to you
  • Coverage is available for you, your spouse, and dependent children
  • Coverage may be continued (with certain stipulations). That means you can take it with you if you change jobs or retire.
  • Plan pays $50 for completing a covered health screening test. This benefit is not paid for dependent children. Click here to view the list of covered health screening tests.
