Cybercriminals continue to utilize social engineering techniques to obtain employee credentials to access payroll information. Cybercriminals are targeting employees through phishing emails to obtain login credentials; therefore, it is essential that you contact the service desk immediately if you receive a phishing email to protect your private demographic information. It is not uncommon for a phishing attack to be targeted towards your UKG/UltiPro credentials with an attempt to access your bank account information, so please continue to be vigilant.
It is important that you review your retirement deferral and fund selection annually to ensure your retirement allocations are on track to meet your retirement goals. As a reminder, please ensure your beneficiary information is up to date with your (403(b) and/or 401(k) plans. To review and/or to make a change, please visit www.standard.com/retirement.
Additionally, the 2026 4% match allocation will be made by March 31, 2027 for employees who have met the eligibility criteria. The match allocation will be denoted on your Standard account and will not be visible in your UKG record.

After Hours Access
To qualify for reimbursement, services must be rendered after-hours between 6:00 pm on Friday through 9:00 pm on Sunday and be for one of the following services:

Voluntary Long-Term Disability
This cost is covered by employer for all salaried employees.
Long Term Disability Insurance provides monthly benefits if you have an illness or injury that leaves you disabled. Benefits from this plan begin after you have been disabled for 90 days. Payments continue until you recover or reach retirement age. During your new hire period only, you can elect coverage, up to the guaranteed issue amount, without answering medical questions!

Even with health insurance, a stay in the hospital can quickly become costly as out-of-pocket expenses begin to add up. This policy is designed to work in conjunction with your medical plan as benefits are paid on top of what your health insurance may cover and can be used at your own discretion. You have two plan options so you can be sure you have the level of protection you need.

Accident Insurance
Having an unexpected accident can cause more than physical injury, it can hurt your bank account, too. Accident Insurance can help cover the out-of-pocket costs associated with on- and off-the-job accidents by paying you a benefit depending on the injuries you suffer and the treatment you receive. Coverage applies towards events such as concussions, dislocations, fractures, physical therapy, and x-rays. This policy is designed to work in conjunction with your medical plan as benefits are paid on top of what your health insurance may cover and can be used at your own discretion.
Critical Illness
No one saves to get sick, which is why being diagnosed with a condition can be especially draining, both emotionally and financially. This policy provides you with a lump sum cash benefit in the event you or a covered dependent is diagnosed with a covered condition such as cancer, heart attack, or stroke. It can help provide financial protection so you can focus on what’s really important—getting better. You also have the option to select your level of coverage so you can make sure you have the right protection for your family. This policy is designed to work in conjunction with your medical plan as benefits are paid on top of what
your health insurance may cover and can be used at your own discretion.

If you enroll in the High Deductible Health Plan, you can make pre-tax contributions to a Health Savings Account (HSA). This account will enable you to be reimbursed for healthcare expenses on a pre-tax basis. Your HSA Debit Card will be issued through PNC Bank.
2027 CALENDAR YEAR CONTRIBUTION LIMITS

If you choose not to participate in the high deductible health plan, you may want to save money by enrolling in a Flexible Spending Account (FSA). This account will enable you to be reimbursed for healthcare expenses on a pre-tax basis. There are two types of Flexible Spending Accounts:
You may participate in one or both accounts. You may deposit up to $3,400 per year in a Health Care FSA and/or up to $7,500 per year in a Dependent Care FSA. The money you set aside in an FSA must be used during the same calendar year. Any funds remaining at the end of the year must be forfeited.

Life insurance provides basic protection to your survivors in the event of your death. Accidental death and dismemberment (AD&D) benefits are paid if you die or are seriously injured in an accident. AD&D insurance doubles your basic life insurance benefit if you die as the result of an accident. It also pays a benefit for certain types of losses such as the loss of sight, speech, or hearing or the loss of the use of your arms, leg, or hands.
BASIC TERM LIFE AND AD&D INSURANCE
Firelands Health provides you with employer-paid, basic group term life and accidental death & dismemberment (AD&D) coverage at no cost. Your basic group term life and AD&D coverage is:
Included with the basic life insurance is accidental death & dismemberment (AD&D) insurance coverage of the same amount. NOTE: Your amount of insurance will decrease to 50% when you turn age 70.
Supplemental Term Life Insurance
Supplemental Life Insurance is available in addition to your employer-provided basic life. You may also purchase life insurance for your dependents if you purchase additional coverage for yourself. No medical questions are required to newly elect a benefit of $10,000 or to increase your coverage by one level up to $20,000.
Firelands Health believes the retirement of an employee is an occasion for special recognition of that individual's contribution to the organization. The purpose of the retirement plan is to help provide for an employee's financial security during the post-retirement years.
Employees of The Bellevue Hospital should contact The Standard with questions (800-858-5420) and to call out anything specific currently noted to reflect Firelands Regional Medical Center and Firelands Physician Group.

Short Term Disability Insurance can help provide benefits to replace a portion of your lost income if you have a sickness or injury and are unable to work.
Employer-Sponsored Short-Term Disability
Firelands Health will provide an employer-sponsored STD plan that covers 60% of income, not to exceed $1,800 per week.
Voluntary Short-Term Disability Buy-Up Plan
You also have the option to purchase additional STD coverage. You can elect to purchase an additional 40% weekly benefit, up to a maximum of $3,000 per week.
