What You Need to Know

This is a passive enrollment, meaning if you do not take action your 2023 benefits will automatically roll over for 2024 with the exception of FSA, LPFSA, and DCFSA.

Plan Updates! Medical Plan ▼

We strive to continue making enhancements to our medical plans. This year we are excited to announce an increase in contributions made by Fisher-Titus for the Tier 1 medical plans. Fisher-Titus will now pay 90% for Tier 1 coinsurance coverage for both the HDHP and Traditional PPO Plans. You only pay 10%! There are updates to prescription drug coverage copays.

We are excited to announce two dental plan enhancements. Enhanced dental plan benefits for special healthcare needs and esthetic restoration coverage.

New! Voluntary Vision Plan ▼

We have partnered with Delta Dental to provide a Voluntary Vision plan with VSP. Dentists and eye doctors are both trained to detect early signs of many conditions, which can lead to earlier intervention and better overall health. In fact, eye doctors are first to report diabetes 34 percent of the time, high cholesterol 62 percent of the time and hypertension 39 percent of the time.

New! Pet Insurance ▼

Spot pet insurance plans reimburse up to 90% of eligible vet bills. As a Fisher-Titus employee you are eligible to get up to a 20% discount off of your policies. See page 33 for more information on how to sign up for the Pet Insurance Plan. This benefit will not be included in the UltiPro 2024 Benefits module and is a direct bill option.

There are two ways to change or elect benefits for 2024:

  • Enroll Online
    • At Work: Log in to the Intranet, under Quick Links, select Ultipro.
    • At Home: Visit www.office.com/apps. Enter system username/password. Enter cell phone for authentication. Enter code texted to phone. Click on Ultipro App from page of apps. Microsoft Edge is the preferred browser.
  • With the Support of a Benefits Counselor
Speak with a benefits counselor to learn more about your benefit options. During your individual appointment, the benefit counselor will:
  • Review your benefits package
  • Answer questions and provide additional information
  • Enroll you in your benefit selections

Counselors are available onsite in the IS Training room. Please be prepared to confirm your Ultipro password, beneficiary(s), and dependent information.

Medical & Prescription ▼

Administered by Medical Mutual

Choose from two medical plans: the High Deductible Health Plan or the Traditional PPO Plan. Both plans are administered by Medical Mutual of Ohio (MMO). Be sure to use Fisher-Titus ( Tier 1), North Coast Healthcare Collaborative (NCHC) (Tier 2), or other in-network providers to receive the highest level of benefits and save money.

Great news! Fisher-Titus will now pay 90% for Tier 1 coinsurance coverage for both the HDHP and Traditional PPO Plans. You only pay 10%! There are updates to prescription drug coverage copays.

The High Deductible Health Plan (HDHP) option will have a higher deductible than the Traditional PPO plan, but the lowest per pay contributions. The HDHP plan also allows for employees to set aside pre-tax savings for health care related expenses into a Health Savings Account. 

The Traditional PPO Plan offers our employees lower deductibles and provides copays for many office-related services compared to the High Deductible Health Plan (HDHP). Contributions for this plan will be higher than those of the HDHP.

Prescription Coverage
Administered by Express Scripts

When you enroll in a Medical Plan, you automatically have coverage for the prescription drug coverage plan. This coverage allows you to fill your prescriptions at the Fisher-Titus pharmacy, Walgreens’ Advantage Network or by Express Scripts mail order. CVS is an excluded pharmacy. For a complete listing of participating pharmacies; please visit My Health Plan. Coverage for Compound and “Discretionary” drugs are excluded from the Prescription Plan.

Under the HDHP, prescription drugs are paid just like other medical services. When you have a prescription filled, you pay the full negotiated cost of the medication until you meet your HDHP deductible (which includes both medical and prescription). Once you meet your HDHP deductible, you pay for prescriptions under the same schedule as the Traditional PPO Plan.

Your cost is determined by where you purchase your prescription and the category of drug. As in the past, in most cases, you will pay the lowest cost-share for prescriptions filled at the Fisher-Titus pharmacy. There is a separate deductible for the Traditional PPO Plan - $200 Single, $400 Family.

Wellness ▼

Fisher-Titus has partnered with Medical Mutual to offer the Fisher-Titus Wellness Program for employees enrolled in the medical plan. The expanded wellness activities offered are aimed at keeping you healthy and reducing the need for medical care now and in the future. Identifying health issues early through preventative care screenings can lead to earlier intervention, easier and lower cost treatment options, and a positive outcome for your health - improving your overall health and wellbeing!

Vision Discount Program ▼

Preferred Provider NOMS/North Central Eye
Fisher-Titus employees and their immediate family receive a 20% discount on a complete pair of glasses when visiting Dr. Zahler and Dr. Allen, in Med Park III. Fisher-Titus health insurance provides one annual routine eye exam under the wellness check that carries with it no out of pocket cost. You can receive a complete eye exam from a trained ophthalmologist. Employees will need to provide proof of employment to receive the discount, such as their badge or insurance card.

Flexible Spending Account (FSA) ▼

Administered by Medical Mutual

Health Care Flexible Spending Account (HCFSA)
This program, available to employees who elect a Traditional PPO Health Plan, allows employees to pay for certain IRS-approved medical care expenses not covered by their insurance plans with pre-tax dollars. Any unused funds in 2023, up to a maximum of $610, may be rolled over to the following year. You have until March 31, 2024 to submit paper reimbursement claims for expenses incurred through December 31, 2023. Substantiation of purchases will be required in most cases in accordance with IRS guidelines. Unused funds in 2024 up to $640, may be rolled over to the following year. Maximum contributions for 2024 is $3,200.

Limited Purpose Flexible Spending Account (LPFSA)
If you choose to elect the HDHP with an HSA, you’re eligible to enroll in a LPFSA. While similar to an HCFSA, money may be set aside in a LPFSA for eligible dental and vision expenses only. Any unused funds in 2023, up to a maximum of $610, may be rolled over to the following year. You have until March 31, 2023 to submit paper reimbursement claims for expenses incurred through December 31, 2023. Unused funds in 2024 up to $640, may be rolled over to the following year. Maximum contributions for 2024 is $3,200.

Nutrition Counseling ▼

The Fisher-Titus medical plan currently provides diet counseling for adults at higher risk for chronic diseases per ACA guidelines; also for children or adults already overweight/obese and have a cardiovascular disease disorder. These must be prescribed by a physician and must be furnished by in network provider. These individuals are eligible for up to 9 visits per year covered at 100%.

Employees and their dependents covered under the Fisher-Titus medical plan will now have access to Nutrition Counseling provided at Fisher-Titus by our qualified dieticians. Employees and their covered dependents will be able to self-refer for up to 2 visits covered at 100%. The nutrition counseling will help patients set nutrition goals to improve their health.

The dietician will:

  • Review your eating habits and lifestyle
  • Do a thorough assessment of your nutritional status
  • Help you create a personalized nutrition treatment plan

Pet Insurance ▼

If you are a pet parent, your pets are important to you and you want to do the best you can to take care of them. 

Attached is the flyer for Spot Pet Insurance in case it is helpful in this situation.  But vet visits can be very expensive.  That is why we are offering pet insurance administered by Spot Pet Insurance.  Pet Insurance is a financial safety net in case of accidents, injuries and illnesses that your pet may experience.

Family and Medical Leave ▼

The FMLA entitles eligible employees of covered employers to take unpaid, job-protected leave for specified family and medical reasons with continuation of group health insurance coverage under the same terms and conditions as if the employee had not taken leave. Eligible employees are entitled to:

  • Twelve workweeks of leave in a 12-month period for:
    • The birth of a child and to care for the newborn child within one year of birth;
    • The placement with the employee of a child for adoption or foster care and to care for the newly placed child within one year of placement;
    • To care for the employee’s spouse, child, or parent who has a serious health condition;
    • A serious health condition that makes the employee unable to perform the essential functions of his or her job;
    • Any qualifying exigency arising out of the fact that the employee’s spouse, son, daughter, or parent is a covered military member on “covered active duty;” or
  • Twenty-six work weeks of leave during a single 12-month period to care for a covered servicemember with a serious injury or illness if the eligible employee is the servicemember’s spouse, son, daughter, parent, or next of kin (military caregiver leave).

Personal Leave of Absence ▼

Purpose:  Fisher-Titus recognizes there are times when an employee may need time off in addition to CTO to address personal matters. For that reason, Fisher-Titus may consider a personal leave of absence without pay as outlined within this policy. If appropriate and approved, this leave also may be available for employees who are ineligible for Family Medical Leave (FML) or who need time off in addition and/or following exhaustion of an FML leave of absence. The approved leave cannot exceed company length of service.

Disability ▼

Core Short-Term Disability Insurance
Administered by Lincoln Financial Group

Core Short-Term Disability Insurance is provided by Fisher-Titus to eligible employees to provide income in the event they should become temporarily unable to work due to injury or illness. This plan may provide up to 60% of an employee’s base salary after they have completed an elimination period of 30 days. Under the Core Short-Term Disability Plan, Short-Term benefits are available up for up to 22 weeks. There are no preexisting conditions under the Core Short-Term Disability Plan.

Buy-Up Short-Term Disability Insurance
Administered by Lincoln Financial Group

A voluntary Buy-Up Short-Term Disability Plan is available to eligible employees. The eligible employee receives the benefit on the 8th day of their disabling injury or illness. If an employee enrolls for the Buy-Up Plan, Short-Term Disability benefits are available for up to 26 weeks. Pre-existing conditions will apply.

Long-Term Disability
Administered by Lincoln Financial Group

LTD coverage is intended to replace a portion of your monthly earnings if you are totally disabled for an extended period of time and are unable to work. Eligible employees will receive 60% of monthly predisability earnings up to a maximum of $10,000 per month after you have been disabled for 180 days (accumulated within a 360 calendar day period). Benefits will continue as outlined in the Maximum Benefits Period in the Summary Plan Description (SPD). Applications for LTD benefits must be approved by the insurance carrier prior to any benefits payments. Your LTD benefit payment will be offset by any money received from Social Security, Worker’s Compensation, or other statutory benefits. The definition of disability for your LTD benefits will be your own occupation for the first two (2) years and benefits for self-reported disabilities will be limited to 24 months. Fisher-Titus pays the cost of this insurance for you.

Tax Consequences of LTD Benefit:
Every year, covered employees have the option of having the appropriate taxes applied to the employer-paid premium, as taxable income. If this option is selected; any LTD benefits payable will not be taxable income. If the employee chooses not to have the appropriate taxes applied to the employer paid premium; taxes will be applied at the time of any LTD benefit paid to the employee. If you wish to change your tax choice election, open enrollment is the one-time-a-year opportunity to do so. Employees must go through the open enrollment election process to make this change. The change would be effective January 1, 2024.

Fisher-Titus offers the 401(k) Salary Savings Plan to help employees save for their retirement. This generous retirement savings plan may offer special tax advantages and the opportunity to receive matching contributions from Fisher-Titus to make saving even easier. You have the option to defer pre-tax (Traditional) and after-tax (Roth) or a combination of both.

Take Advantage of the Company Match
100% on the first 2% of pay you defer and 50% on the next 4% of pay you defer. In other words, when you defer 6% of your pay, Fisher-Titus will contribute 4% of your pay. This equates to a total contribution of 10% of pay!

Employee Assistance Program ▼

The EAP is for anyone with questions or concerns – no matter how big or small they might be.

  • The Employee Assistance Program is designed to help Fisher-Titus employees resolve personal problems.
  • The EAP provides problem assessment, referrals, short-term counseling, education, and convenient appointments. It offers confidential assistance from a counselor and other professionals to assist in finding resolutions to problems that may be affecting your job or personal life.
  • All information relayed in confidence will remain just that – confidential.
  • Employees may contact the EAP on their own or HR would be glad to assist you.

Child Care Resource Center - NEW! ▼

Fisher-Titus understands the need for childcare in the workplace is an important issue for employees who have dependent children at home.

To that end, Fisher-Titus is pleased to share that the Child Care Resource Center will offer the following services to our employees to help address some of the needs identified by our employees:

Enhanced Child Care Referral Services: Customized searches for legal childcare options based on family criteria, including:
    • A minimum of 3 referrals to childcare programs.
    • Detailed profiles of each referred provider/program.
    • Consumer education materials on childcare selection, types of care, and affordability. Tailored information may also be provided on specific childcare needs such as special needs care, relative care, or family childcare.
    • Access to a CCRC Parent Specialist for guidance, support, and additional resources throughout the search process.
  • Workplace Lunch and Learn Series: Brief sessions addressing topics identified by staff, providing resources, support, and connections to community or regional supports.
  • Parent Café: Small group sessions led by trained Parent Hosts, allowing parents to share and learn from each other on topics such as self-care, effective parenting, and building strong relationships with children.
  • Foster Care Supports and Connection: Comprehensive assistance for those considering kinship, foster, or adoptive parenting. Free services are available to help navigate each step of the process, ensuring safe, supportive, and loving homes for children.

Adoption Assistance Program ▼

The Adoption Assistance Program provides financial support to eligible Fisher-Titus employees who are adopting a child. The program is available for regular fulltime and part-time employees (as defined in the Employment Classification Policy). Please review the Adoption Assistance Policy for additional details.

Fisher-Titus Facility Fee Waived ▼

Employees and their legal spouse, who are enrolled in the Fisher-Titus medical plan and choose to deliver their baby at Fisher-Titus Medical Center, will have the facility portion of the delivery covered at 100%. This represents a considerable savings for our staff who entrust their maternity care to our outstanding maternity team here at Fisher-Titus. Individuals eligible for this benefit will still be responsible for delivery related professional fees, such as physician and anesthesia services.

Enhanced Paid Parental Leave - NEW! ▼

Fisher-Titus will provide up to two regularly scheduled work weeks of paid parental leave to employees following the birth of an employee’s child or the placement of a child with an employee in connection with adoption or foster care. The purpose of paid parental leave is to enable the employee to care for and bond with a newborn or a newly adopted or newly placed child.

Breast Feeding Support ▼

Fisher-Titus has a caring team of Birthing Center staff that support moms in our community with their breastfeeding questions and concerns. All of our Birthing Center staff have had additional training to assist breastfeeding families, with several staff nurses advancing their education to become Certified Breastfeeding Counselors or Certified Lactation Counselors, the premier national certification in lactation management in the U.S. We also have an International Board Certified Lactation Consultant (IBCLC) on staff.

Fisher-Titus has earned recognition from Ohio's First Steps for Healthy Babies, the Maternity Care Best Practice Award, and has been recognized by Newsweek as a Best Maternity Care Hospital for the exceptional care we provide to mothers, newborns, and their families.

Baby Boot Camp ▼

This is an opportunity for the Fisher-Titus Birthing Center staff to assist you and your partner in feeling comfortable, confident, and prepared for your new arrival!

During this one-on-one appointment you will:

  • Tour the birthing center
  • Pre-register
  • Start the discharge process early
  • Prepare for the delivery of your baby
  • Ask any questions you may have about the birthing process
  • Meet with a certified lactation consultant and learn more about breastfeeding

Lactation Support ▼

Are you a mom breastfeeding or pumping to provide milk to your baby? Join us every month for support and guidance along your breasfeeding journey! The group is led by Carrie Daugherty, RN, IBCLC, International Board Certified Lactation Consultant who will answer any questions moms may have and will weigh your baby before and after breastfeeding. This is a free service for breastfeeding moms.

  • Board Certified Lactation Consultants on staff to serve the needs of nursing moms and babies
  • Telephone consultation
  • Visits and support for moms and babies in the hospital
  • Referrals from and to other professional services
  • Educational in-services for professionals
    Prenatal breastfeeding education
    Outpatient lactation appointments

Huron County REACT Peer to Peer Support System ▼

The Huron County Responder Exposure Assistance and Care Team (REACT) is a Huron County Peer support program that is available 24/7 to any Huron County responder, including health care employees, in need.  The REACT team is made up of local first responders who understand the unique challenges faced by first responders. 

REACT team members have been trained in therapeutic communication, crisis intervention, and community resource networking.  They can provide assistance to both active and retired Huron County responders and health care employees who are struggling and may be experiencing behavioral health and/or addiction issues. 

REACT team members are equipped to provide support to their peers through formal and informal relationships that address psychological, emotional, spiritual, and physical health.  Peer supporters provide emotional support, share knowledge, teach skills, provide practical assistance, and connect people with resources, opportunities, communities of support, and other people.

Team members are available to meet with peers individually and also in group settings when requested.  The team may be contacted to meet with a department, or certain members of a department, after a difficult call or case to help them process the event, provide support, and link them with resources for additional service, if needed.
