OXHP’s PPO Plan offers you a high level of coverage and reduced rates for health care services when you use doctors and other providers in the Freedom network.
Out-of-Network claims are processed and paid at 225% of Medicare’s reimbursement schedule. This is a change from last year, so please note that you will have more out-of-pocket expenses if you visit out-of-network providers. Out-of-Network providers are allowed to balance bill you for all amounts over the reimbursement level. The amount over the reimbursement level does not accumulate towards your deductible or out-of-pocket maximum.
Coordinating in network testing facilities, laboratories, and supporting physicians is easy and can help save you money. Take the time to ask that participating labs, testing facilities and specialists be used whenever you need medical care.

The UHC PPO dental plan offers you the flexibility to visit any dentist, with higher benefits paid when you visit an in-network provider in the Dental PPO 30 Network.
After you meet the annual deductible, the plan pays a percentage of covered expenses until you reach the annual maximum benefit limit. Your out-of-pocket costs are lower when you visit a network dentist.

Routine eye exams are an important part of preventive care services and can help preserve vision wellness. Eye exams can also aid in detecting other health problems.
UnitedHealthcare’s vision plan helps you get the vision care you need without feeling like you’re exceeding your budget. This PPO-based plan covers most services at 100% (in-network) after applicable copays. There is also out-of-network coverage when you utilize a provider who is not part of United Healthcare’s listing of providers.
