New Hire Enrollment Process

You must complete an enrollment appointment with a professional benefit counselor to elect coverage. You must take action by the first of the month following 60 days of employment. During your confidential appointment, the benefit counselor will:

  • Answer your questions
  • Provide detailed information
  • Help you enroll in benefits

Even if you don’t wear glasses, regular visits to your eye doctor are important to your overall health. That’s because a routine eye exam can often detect early signs of serious health conditions like diabetes.  A group vision plan makes it easier to get the care you need and lower your costs. 

Vision insurance helps offset the cost of routine eye exams and helps pay for vision correction eyewear like eyeglasses and contacts by an eye-care provider. You’re eligible for an eye exam and lenses or contact lenses every 12 months and frames every 24 months. Out-of-network providers will only offer you an allowance towards your vision services.

Voluntary Long-Term Disability

Long-term disability insurance is a type of income protection that is designed to cover serious injuries and illnesses that keep you out of work for an extended amount of time. There is a pre-existing condition exclusion – any condition you’ve been treated for or diagnosed with for 12 months before your effective date of coverage will be excluded for 24 months after effective date of coverage.

  • Waiting Period: Begins on the 91st day of continuous injury or illness
  • Benefit Amount: 60% of monthly earnings
  • Maximum Benefit: $10,000 per month
  • Length of Payment Period: SSNRA
  • *Pre-Existing Condition Exclusion: 12/24
  • Premium Contribution: Employee paid

Voluntary Short-Term Disability

If you become ill or suffer a non-work-related injury that prevents you from working short-term disability insurance provides income replacement. There is a pre-existing condition exclusion – any condition you’ve been treated for or diagnosed with for 12 months will be excluded from coverage for 12 months after the policy's effective date.

  • Waiting Period: Begins on the 15th day of continuous injury or illness
  • Benefit Amount: 60% of weekly earnings
  • Maximum Benefit: $5,000 per week
  • Length of Payment Period: 3 months
  • *Pre-Existing Condition Exclusion: 12/12
  • Premium Contribution: Employee paid

Basic Life and AD&D Insurance
Life insurance helps ease your loved ones’ financial burden. Your designated beneficiary will receive a benefit if you pass away from a covered accident or illness. In addition, the Accidental Death and Dismemberment (AD&D) benefits paired with life insurance provide a benefit to your beneficiary if you pass on or become dismembered due to a specifically covered accident. Benefits may reduce as you age. See your plan documents for more details.

This benefit is 100% company-paid, and you receive it if you elect medical coverage.

Voluntary Life Insurance
Voluntary term life/AD&D allows you to purchase additional life and AD&D coverage for yourself and your dependents. Your age and the amount of insurance you elect determines the premium you’ll pay. Costs will increase as you age, and benefits may be reduced. See your plan documents for more detail and to determine if evidence of insurability applies. This is your one-time opportunity to elect up to the Guarantee issue limits for voluntary life insurance – please refer to the benefit summary for specific limits.

Cancer Advocate Plus (CAP) is a personal, precise, and proactive way for you to manage your health. CAP combines Financial Protection and Cancer Care, including Genetic Cancer Screening, to help you determine your risk for cancer and Pharmacogenomic Testing to determine your best treatment based on your own DNA.
