New Hire Enrollment Process ▼

Professional benefit counselors will be available over the phone to help you complete enrollment. During your confidential meeting, the counselor will:

  • Review your benefits and help you understand the programs available to you
  • Answer your questions and provide additional information
  • Help you customize a benefits package to best meet your individual needs
  • Complete the enrollment process for yourself and your dependents

1. General & Enrollment

    • Q: When can I enroll in benefits?
      • A: New employees will be scheduled a benefits call to enroll, about a month after hire. Benefits become effective for all new hire employees the 1st of the month, following 60 days of employment. Open enrollment occurs annually for changes to take effect on the first day of the new plan year.
    • Q: What happens if I miss the open enrollment deadline?
      • A: If you miss the deadline, you will generally not be allowed to make changes to your benefits until the next open enrollment period, unless you experience a qualifying life event.
    • Q: When does Open Enrollment take place?
      • A: Open enrollment calls take place in October/November each year, for a benefits effective date of January 1st.
    • Q: What is a Qualifying Life Event (QLE)?
      • A: A Qualifying Life Event (QLE) is a significant life change—such as marriage, divorce, birth/adoption of a child, or loss of other coverage—that allows you to change your benefits outside of open enrollment.
    • Q: What do I do if I have a QLE or a status change, and I need to enroll or make changes?
      • Visit your HR Director to provide your documentation (for QLEs), and to obtain a Benefits Election form. Complete the form and return it to your HR Director for enrollment.
    • Q: Who is an eligible dependent?
      • A: Eligible dependents include your legal spouse, domestic partner, and children (up to age 26).

2. Health, Dental, and Vision Insurance

    • Q: How much do benefits cost?
      • A: Please review our Benefits Guide, or our Benefits Microsite for more information on coverage and plan pricing.
    • Q: What is a Minimum Essential Coverage (MEC) plan?
      • A: A Minimum Essential Coverage (MEC) plan is a low-cost health insurance option that satisfies the Affordable Care Act's (ACA) requirement for having coverage, focusing primarily on preventative services
    • Q: What is the difference between a deductible, co-pay, and co-insurance?
      • A: A deductible is the amount you pay for services before insurance begins to pay. A co-pay is a fixed amount you pay for a service (e.g., $20 for a doctor visit). Co-insurance is your percentage share of the costs of a service (e.g., 20%) after the deductible is met.
    • Q: How do I know if my doctor is in-network?
      • A: You can check Leading Edge’s website, or call the doctor’s office to ask if they accept your specific insurance plan.
    • Q: Where can I find my ID cards and plan information?
      • ID cards, coverage information, and claim details can be found on the carrier portals. See your HR Director for registration instructions.
    • Q: Can I enroll in the Health Savings Account (HSA) without enrolling in the HSA medical plan?
      • No, you must be enrolled in the HSA medical plan to open and enroll into an HSA bank account.
    • Q: When can I cancel my medical, dental and vision coverage?
      • A: Medical, dental, and vision coverage can only be cancelled during open enrollment each year; unless there is a qualifying life event.

3. Voluntary Benefits

  • Q: Where can I find claim forms for my voluntary policies?
    • A: Claim forms can be found on respective carrier portals. Links to these sites can be found on our Benefits Microsite. Claim forms can also be obtained through your facility HR Director.
  • Q: Can I cancel my voluntary benefits?
    • A: Yes. Voluntary benefits can be cancelled at any time, for any reason. See your facility HR Director to obtain the Voluntary Benefits cancellation form.
  • Q: Are my voluntary benefits portable?
    • A: Yes. This means – if you leave the company, your policies will stay with you. The carrier will reach out to you directly to coordinate the shift to an individual policy.

4. Benefits & Leaves of Absence

  • Q: Will my benefits remain active if I go on LOA or FMLA?
    • A: Yes. Your benefits will not be cancelled due to going out on an approved leave of absence, or FMLA.

  • Q: Am I required to pay for my benefits while I am on leave?
    • A: Benefit premiums will be deducted from sick and/or vacation hours that are paid out while you are on leave. We don’t require you to pay for premiums while not working – however, you will be responsible for repaying any missed premiums when you return.
  • Q: How do I repay my benefit premiums when I return from leave?
    • A: Check or money order – payable to your facility; payment mailed or delivered to Legacy Corporate (3450 Oakton St. Skokie, IL 60076) – attention Jamie. OR- catch-up premiums can be deducted through payroll. Please see your facility HR Director to arrange this. Voluntary benefit premiums will be billed to you and repaid directly through the carrier.

5. Leaving the Company

    • Q: What happens to my benefits if I leave the company?
      • A: Health coverage ends on your last day of work (if January 15th was the last day worked, then January 15th would be the last covered day). You may be eligible to continue coverage under COBRA, which allows you the option to continue medical, dental, and vision coverage for up to 18 months.
    • Q: Can I keep my HSA money?
      • A: Yes, your Health Savings Account (HSA) and all the money in it belong to you, and does not expire.

Glossary of terms: Benefits ▼

    • 401(k) / Defined Contribution Plan: A retirement plan allowing employees to contribute pre-tax income, often with an employer match.
    • Coinsurance: Your percentage share of the cost for covered services after meeting your deductible.
    • Contingent (Secondary) Beneficiary: A "backup" who inherits only if all primary beneficiaries have died, cannot be located, or refuse the inheritance
    • Copayment (Copay): A fixed fee paid at the time of service for specific types of care.
    • CORE Coverage: Medical, dental, and vision benefits
    • Deductible: The amount you pay out-of-pocket annually before your insurance begins to cover costs.
    • Dependents: an individual—most commonly a spouse or child—whom a primary policyholder (employee) is eligible to add to their insurance or benefit plans.
    • Domestic Partner: an individual in a committed, long-term relationship with an employee where both parties share a common residence and financial obligations but are not legally married.
    • Election Form: (or enrollment form) is a legal document used by an employee to formally select or decline specific workplace benefits
    • Explanation of Benefits (EOB): A document from the insurer detailing billed services, covered amounts, and remaining balances.
    • HSA (Health Savings Account): A portable account for individuals with an HDHP, offering tax advantages; funds roll over and remain with the employee.
    • Medicaid: a joint federal and state program that provides free or low-cost health coverage to individuals and families with limited income and resources.
    • Medical Support Order: a legal mandate, typically issued by a court or state administrative agency, that requires a parent to provide health insurance coverage or financial medical support for their child.
    • Medicare: a federal health insurance program primarily for individuals aged 65 or older. It also provides coverage for younger people with specific disabilities, End-Stage Renal Disease, or ALS.
    • Minimum Essential Coverage (MEC): refers to a health insurance plan that meets the basic standards established by the Affordable Care Act (ACA).
    • Open Enrollment (OE): the designated annual window during which individuals can sign up for, adjust, or cancel their health insurance and other employee benefits
    • Out-of-Pocket Maximum: The most you will pay in a plan year for covered, in-network services; the insurer pays 100% after this limit is met.
    • Premium: The regular payment to maintain health insurance coverage.
    • Primary Beneficiary: The first person or entity in line to receive the death benefit or account balance. You can name multiple primary beneficiaries and specify the percentage of the total benefit each should receive.
    • QLE (Qualifying Life Event): An event (e.g., marriage, birth) that permits changes to benefit elections outside of the standard Open Enrollment period.
    • Supporting Documentation: the essential legal or financial records required to verify an individual's eligibility for specific employee benefits or to substantiate a claim for reimbursement.
    • TPA (Third Party Administrator): an external service provider that handles the day-to-day administrative and operational tasks for an organization's employee benefit plans.
    • Voluntary Benefits: Additional benefits like life, accident, and disability insurance.
    • Wellness Benefit: an employer-sponsored program or initiative designed to promote and maintain the physical, mental, emotional, and financial well-being of employees.

Medical & Pharmacy ▼

Legacy Healthcare offers four medical plans through Leading Edge with Anthem, a BCBS Network. These plans offer the flexibility to see any doctor you wish; however, you will pay less out-of-pocket when you see an in-network provider. Pharmacy Benefits
Your drug plan is administered by Carelon. Visit Carelon at carelon.com to search for prescription drugs, find a pharmacy, order mail service refills or new prescriptions online, and much more. Carelon has a broad network of contracting pharmacies. To use your benefits, simply find a contracting pharmacy close to you and present your member ID card.

Rx Mail Order
The Mail Order program with Carelon will save you both time and money. This program requires all covered medications to be filled as a 90-day supply either through select retail pharmacies or through home delivery. Only two 30-day fills are permitted at a retail pharmacy before you are required to move to a 90-day supply.

Short-Term Disability ▼

Short-Term Disability (STD) from Allstate provides partial salary replacement if an unexpected illness or out of workplace injury keeps you from working. You can customize your plan by selecting when benefits begin and how long they last. Short-Term Disability can help fill the gap until you are able to return to work by paying you a weekly benefit. This policy pays a benefit of 60% of your salary up to a maximum benefit of $1,250 a week. You have the option of customizing your coverage by selecting when your benefits begin after your disability, as well as the duration of your benefit.

Accident Insurance through MetLife can help cover the out-of-pocket costs associated with an accident (24/7/365) by paying you a benefit depending on the injuries you suffer and the treatment you receive. You can use the money as you see fit, whether to pay for expenses associated with your accident, like an emergency room copay, or to pay for childcare so you can get to the doctor for a follow-up visit. During this enrollment period only you can elect coverage without answering medical questions!

The policy does not coordinate with any other coverage, so you can still receive benefits on top of what your medical plan provides. Accident Insurance pays you a benefit for events such as ambulance transportation, ER visits, doctor visits and follow-up treatment, fractures and dislocations, burns, surgery, medical appliances, torn cartilage, and more.

If you enroll in the HSA medical plan, you are eligible for a Health Savings Account. Legacy will manage your pre-tax payroll deductions for you if you chose to contribute to your account. An HSA is an interest-bearing savings account used to pay for eligible medical, prescription drug, dental, or vision expenses. All the money in the account is yours to keep, even if you leave or retire from Legacy. At the end of the year, any money in the HSA rolls over to the next year—you never lose it! The 2026 annual maximum contribution is $4,400 for an individual or $8,750 for a family.

This enrollment period is the only opportunity to enroll in benefits until the next annual enrollment period unless you experience a qualifying life event.

What is a qualifying life event?

Certain coverages allow limited changes to your benefit elections during the year. These benefits include the Medical, Dental, Vision, Life, and Disability. For these benefits, you may only make changes to your elections during the year if you have a qualifying life event.

Qualifying life events include:

  • Change in legal marital status, including marriage, divorce, death of a spouse, legal separation, or annulment
  • Change in number of children, including birth, adoption, placement of adoption, or death of a child
  • Change in employment status of employee, spouse, or dependent, including termination or commencement of employment, commencement or return from unpaid leave of absence
  • Change in dependent eligibility status, including attainment of age, student status, or any similar circumstance

The change to your benefit elections must be consistent with the qualifying life event. You have 30 days from the qualifying life event to request your enrollment change.
