New Hire Benefit Information ▼

The benefits you elect will be effective through May 31, 2024, unless you experience a qualifying life event. Such events include birth, death, marriage, divorce, adoption, or a change in status, so choose your benefits carefully. If you have a qualifying event, you must notify HR within 31 days to change your benefit elections.
You must speak with a benefit counselor to waive or enroll in coverage. During your personalized individual meetings, your benefit counselor will:

  • Review your benefits and help you understand the programs available to you
  • Answer your questions and provide additional information
  • Help you customize a benefits package to best meet your individual needs
  • Complete the enrollment process for yourself and your dependents

Because maintaining your smile is important, Legacy offers two dental plans through MetLife. Although you have the option to see any provider you wish, you will receive the best benefits when you choose an in-network dentist. Preferred Provider Organization (PPO) – You have the flexibility to use any dentist of your choice, however, you
can manage your out-of-pocket costs by remaining in-network. Negotiated fees extend to all in-network services— even to non-covered services like cosmetics and to services provided after the annual benefit maximum has been exceeded. Dental implants are covered under both dental plans. Orthodontia is covered under both plans for Children up to 19 years of age.

On January 1, 2024, all members will receive a refreshed Dental Annual Plan Maximum amount as the new calendar year begins.

Short-Term Disability ▼

Short-Term Disability (STD) from Sun Life provides partial salary replacement if an unexpected illness or out of workplace injury keeps you from working. You can customize your plan by selecting when benefits begin and how long they last. Short-Term Disability can help fill the gap until you are able to return to work by paying you a weekly benefit. This policy pays a benefit of 60% of your salary up to a maximum benefit of $1,250 a week. You have the option of customizing your coverage by selecting when your benefits begin after your disability, as well as the duration of your benefit.

Accident Insurance through Chubb can help cover the out-of-pocket costs associated with an accident (24/7/365) by paying you a benefit depending on the injuries you suffer and the treatment you receive. You can use the money as you see fit, whether to pay for expenses associated with your accident, like an emergency room copay, or to pay for childcare so you can get to the doctor for a follow-up visit. You can elect coverage without answering medical questions during this enrollment period only.

The policy does not coordinate with any other coverage, so you can still receive benefits on top of what your medical plan provides. Accident Insurance pays you a benefit for events such as ambulance transportation, ER visits, doctor visits and follow-up treatment, fractures and dislocations, burns, surgery, medical appliances, torn cartilage, and more.

Critical Illness Insurance ▼

Critical Illness Insurance from Aetna protects your family and your assets. No one saves to get sick, which is why being diagnosed with a covered condition can be especially draining, both emotionally and financially. The policy provides you with a lump sum cash benefit in the event you or an insured loved one is diagnosed with a covered condition such as cancer, heart attack, stroke, or infectious disease, such as COVID-19 or pneumonia. It can help provide financial protection so you can focus on getting better. The policy also includes a $50 annual health screening benefit when you complete a qualified screening such as a COVID-19 test. You can elect coverage without answering medical questions during this enrollment period only.

Health Savings Account (HSA) ▼

If you enroll in the BCBS PPO HSA medical plan, you are eligible for a Health Savings Account (HSA). Legacy will manage your pre-tax payroll deductions for you if you chose to contribute to your account. An HSA is an interest-bearing savings account used to pay for eligible medical, prescription drug, dental, or vision expenses. Over-the-counter drugs and menstrual products are now eligible expenses under the HSA!

Telehealth expenses are now eligible expenses under an HSA. For HSA purposes, these expenses are covered without requiring that the HDHP deductible be met.

All the money in the account is yours to keep, even if you leave or retire from Legacy. At the end of the year, any money in the HSA rolls over to the next year—you never lose it! The 2022 annual maximum contribution is $3,650 for an individual or $7,300 for a family.

Even with health insurance, a stay in the hospital can become costly quickly as out-of-pocket charges begin to add up. Hospital Indemnity Insurance can reduce the financial and emotional stress of a hospital stay by providing a lump sum cash benefit directly to you that can be used however you need, whether that’s for coinsurance or childcare. 

A Simplified Claims Experience

  1. Register on the My Aetna Supplemental app or on the member portal at Myaetnasupplemental.com to submit and track claims, and sign up for direct deposit.

Basic Life and Accidental Death & Dismemberment (AD&D) Insurance helps provide a foundation of financial protection for your family if something happens to you.

What is Group Basic Life Insurance?

Legacy provides 1x your salary up to $50,000 of life and AD&D coverage to all eligible employees. Group Term Life Insurance pays a benefit to your chosen beneficiary(ies) if you die while you are a member of the group covered by the policy. It does not earn interest and has no cash value but can help protect the financial future of your loved ones.

Group AD&D pays a benefit in addition to your life insurance if you die as the result of a covered accident. It also pays benefits for loss of limb, hearing, speech, sight, and other serious losses resulting from an accident. See your policy documents for details of coverage.

Legacy Healthcare is happy to offer Universal Life Insurance with Living Benefits for Long Term Care from Allstate, which features premiums guaranteed for life and coverage for qualified long-term care expenses like a nursing home, assisted living, or home care. Living benefits for long-term care protects your family with money that can be used any way they choose. It is most often used to pay for mortgage or rent, education for children and grandchildren, retirement, family debt, and final expenses.

Don't Miss Out!

This enrollment period is the only opportunity to enroll in benefits until the next annual enrollment period unless you experience a qualifying life event.

What is a qualifying life event?

Certain coverages allow limited changes to your benefit elections during the year. These benefits include the Medical, Dental, Vision, Life, and Disability. For these benefits, you may only make changes to your elections during the year if you have a qualifying life event.

Qualifying life events include:

  • Change in legal marital status, including marriage, divorce, death of a spouse, legal separation, or annulment
  • Change in number of children, including birth, adoption, placement of adoption, or death of a child
  • Change in employment status of employee, spouse, or dependent, including termination or commencement of employment, commencement or return from unpaid leave of absence
  • Change in dependent eligibility status, including attainment of age, student status, or any similar circumstance

The change to your benefit elections must be consistent with the qualifying life event. You have 30 days from the qualifying life event to request your enrollment change.

Important Terms ▼

  • Coinsurance: A plan’s coinsurance defines how much you pay and how much the plan pays for covered services once the deductible has been met. For example, if a plan’s coinsurance is 70%/30%, the plan pays 70% and you pay 30%. Please keep in mind that your deductible must be met before the plan’s coinsurance will apply in most instances.
  • Co-payment (or Copay): A co-payment, or copay, is a predetermined amount that you must pay out of pocket to a participating provider. The copay is paid directly to the provider and is due at the time services are rendered.
  • Deductible: A deductible is a specified amount that you must pay before the plan will pay a claim. For certain types of services, the deductible is waived and you may only pay a copay at the time of services. Copays are not applied toward the deductible.
  • Exclusions and Limitations: Specific conditions or circumstances for which an insurance policy or plan will not provide coverage (exclusions), or for which coverage is specifically limited (limitations).
  • Guarantee Issue: Some plans have Guarantee Issue. Guarantee Issue is typically provided for life and disability insurance during the initial enrollment offering or when you are electing benefits for the first time as a new hire. Guarantee Issue provides a specified amount of guaranteed coverage without having to receive underwriting approval.
  • Out-of-Pocket Maximum: This is the most you could pay during a calendar year for your share of covered services. Deductibles and copays count toward the out-of-pocket maximum, but balance billing and ineligible expenses do not.
  • Pre-certification: Approval granted by a case manager or insurance company representative for a person to receive certain services such as inpatient hospital admittance or outpatient surgery. The goal is to ensure that individuals are not exposed to inappropriate health care services or services that are not medically necessary.
  • Qualifying Life Event: Certain coverages allow limited changes to your benefit elections during the year. These benefits include the Medical, Dental, Vision, Life, and Disability.
