To ensure you understand your benefit options, we strongly encourage you to speak with a professional benefit counselor to enroll in or waive benefits. During your individual appointment, you will have the opportunity to ask questions, get more information, and enroll yourself and your dependent(s) in coverage.

New Universal Life Insurance with Long Term Care ▼

Universal Life Insurance with Long Term Care through Allstate provides lifelong protection with flexible premiums. It is one policy with two benefits - life insurance coverage for your family and long term care benefits if you need them.

As Life Insurance, this plan protects your family with money that can be used any way they choose. It is most often used to pay for mortgage or rent, education for children and grandchildren, retirement, family debt, and final expenses.

Plan Highlights

  • Coverage is available for yourself, your spouse, and your child(ren)
  • Permanent, portable, and flexible coverage
  • Benefits for long term care services such as home health or nursing home care
  • You pay for this program conveniently through payroll deduction
  • During this enrollment period you have a special opportunity to elect coverage without answering medical questions:
    • Employee: up to $100,000
    • Spouse: up to $50,000
    • Child: up to $20,000

How to Enroll

Benefits are more important than ever and we encourage you to spend extra time evaluating your benefit options. As such, we have partnered with EOI Service Company so you can enroll with the guidance of a professional benefit counselor.

A benefit counselor will be available onsite May 15 - 17, 2023 for individual appointments. If you are unable to meet with a benefit counselor onsite, telephonic appointments will be available through May 23, 2023.

Each benefit-eligible employee is encouraged to speak with a benefit counselor to enroll in voluntary benefits for the 2023-24 plan year. During your confidential appointment, the benefit counselor will:

  • Answer your questions
  • Provide detailed information
  • Enroll you and your dependent(s) in your selected benefits
