Sanvello is a mobile app that gives you access to clinically proven techniques for dealing with stress, anxiety, depression, or whatever else you may be going through. From coping tools to Guided Journeys, you’ll get help to manage your moods and thoughts so you can understand what works for you to feel better. Download the app from the App Store or Google Play, create a Sanvello account and then select “Upgrade Via Insurance” and search for and select “Medica.” Enter the information from your Media ID card.
My Health Rewards is a health and wellness program that helps you take small steps to reach your health goals. Earn up to $220 in rewards by engaging in healthy activities. 100% Preventive Care & Medical Information Resources
Preventive care can help keep you healthy or catch a possible problem early. Preventive services generally screen for certain types of cancer, immunize you against disease and are received once per year, or as recommended by your doctor. When you visit a network provider, your plan covers preventive services at 100%. If you receive both preventive and non-preventive care at your visit, the preventive services will be covered at 100%. You’ll pay a share of the costs for the non-preventive services.

Staying healthy includes good dental care. Monarch Healthcare Management’s dental plan provides the comprehensive coverage necessary to help you and your family maintain good dental health. The dental benefit is administered by Delta Dental.
Employees have the choice between two plans; the Low Option and the High Option. The main difference between these plans is that the Low Option does not cover Major services and Orthodontics. Both plans cover preventive care at 100% in-network.

Basic Life and AD&D Insurance
You can’t always predict – or control – your life. But, you can prepare for it. Protecting the financial interests of your loved ones in the event of your death or serious injury can be invaluable. Monarch Healthcare Management provides life and AD&D (accidental death or dismemberment) benefits through MetLife. Eligible employees automatically receive the basic portion of the Life and AD&D benefit – there are no choices to be made. Any benefits paid out are tax-free to the recipient.
Short Term Disability Insurance through Voya provides protection if an unexpected illness or injury keeps you from working. If you become disabled and can’t work, you may face a gap between your income and expenses. Short Term Disability Insurance can help fill this gap by paying you a weekly benefit until you are able to return to work.

Hospital Indemnity Insurance through Voya can reduce the financial and emotional stress of a hospital stay by providing a lump sum cash benefit directly to you that can be used however you need. How does it work?
With Hospital Indemnity Insurance, you’ll receive a fixed daily benefit if you have a covered stay in a hospital that occurs on or after your coverage effective date. Benefit amounts are listed below and depend on the type of facility and number of days of confinement. Any combination of facility confinement and admission benefits payable includes a limit, please see your certificate for further confirmation. And for a complete description of your available benefits, exclusions and limitations, see your certificate of insurance and any riders.

Identity theft protection services from InfoArmor help assess your risk, deter theft attempts, detect fraud, and manage the restoration process in the event of identity theft. If InfoArmor detects suspicious activity, a certified privacy advocate can act as a dedicated case manager on your behalf and resolve the issue.
Monarch Healthcare Management offers a vision plan administered by EyeMed. This coverage is a voluntary benefit that features coverage for prescription glasses and contact lenses, as well as other vision-related items.
You have the freedom to receive services from any provider. You will, however, receive a greater level of benefit if you use a provider who participates in the EyeMed Insight network. By using a network provider, you may also receive discounts for services not otherwise covered by the vision plan (i.e., sunglasses and laser vision correction).
Please note: this plan provides coverage for materials and hardware only. Coverage for routine annual vision exams are provided through your Medica medical plan as a preventive appointment as long as you use an in-network provider.
A Health Savings Account (HSA) is a triple tax-advantaged savings account. If you enroll in either the $3,000-80% Plan B or $6,350-100% HSA Plan C, you can contribute to an HSA on a pre-tax basis through payroll deduction. Annual contributions are limited by federal law depending on the level of health coverage you elect. You can start, stop or change your HSA contribution at any time during the year.

The Medical Flexible Spending Account (FSA) gives participants in the $1,000-80% Plan A the opportunity to set aside pre-tax dollars to pay for qualified medical, dental and vision expenses. Examples of eligible expenses include deductibles and co-pays, prescription drug costs, over-the-counter medicines (if prescribed by a doctor), and other non-covered medical, dental, vision and hearing care expenses. The FSA year is July 1st through June 30th.
What is a Dependent FSA?
The Dependent Care Flexible Spending Account (FSA) allows employees to set aside pre-tax dollars to pay for eligible dependent daycare expenses. Contributions are automatically deducted from your paychecks on a pre-tax basis, saving you money by not paying federal, state or Social Security taxes on the portion of your income that you contribute to the plan. The account is administered through WEX.
Universal Life Insurance through Transamerica offers Monarch Healthcare Management employees working a minimum of 30 hours per week flexible financial protection. There are a number of optional riders, including “living benefits” that allow you to accelerate a portion of the life insurance death benefit if diagnosed by a physician as chronically or terminally ill. These extra “living benefits” can be used to help pay expenses from an unexpected medical crisis and alleviate the worry of future financial problems.

And every year that you complete a qualified health screening, you and your covered dependents are eligible for a $50 Health Screening Benefit.* Cholesterol tests, as well as many other preventative tests, qualify for this health screening benefit.
*The Wellness (Health Screening) Benefit pays the amount shown on the Plan Description for one (1) health screening test performed during a twelve (12) month period for each Insured, up to a maximum of four benefits per family.
No one can be completely prepared when a critical illness strikes, but if you or a loved one is diagnosed with a covered condition such as cancer, stroke, or heart attack, Critical Illness Insurance through Voya provides a lumpsum cash benefit to help pay for out-of-pocket medical expenses or any other bills that need attention, including rent or groceries. And every year that you complete a qualified health screening, you and your covered dependents are eligible for a $50 Health Screening Benefit.* Cholesterol tests, as well as many other preventative tests, qualify for this health screening benefit.
*The Wellness (Health Screening) Benefit pays the amount shown on the Plan Description for one (1) health screening test performed during a twelve (12) month period for each Insured, up to a maximum of four benefits per family.
This new hire enrollment period is the only opportunity to enroll in benefits until the next annual enrollment period unless you experience a qualifying life event and request a change to your benefits within 30 days of the life event.
Certain coverages allow limited changes to your benefit elections during the year. These benefits include the Medical, Dental, Vision, Life, Disability and Flexible Spending Accounts. For these benefits, you may only make changes to your elections during the year if you have a qualifying life event.
