You have the option to open a Health Savings Account (HSA) if you enroll in the HDHP with HSA plan. HSA funds can be used to pay for eligible medical, dental and vision expenses for you and your eligible dependents, including deductibles, coinsurance, prescriptions, acupuncture and more, tax-free, now and in the future. Family members do not need to be covered under your plan in order to use the HSA funds.

FSAs allow you to save money on a pre-tax basis to pay for qualified medical, dental and/or vision expenses you may incur throughout the year. The money you put into your FSA is done so on a pre-tax basis. This means you are lowering your taxable income and also not paying taxes when the money is used for qualified expenses. Health Care FSA is applicable to the two medical PPO plans.
Health Care FSA - You may contribute up to $3,050* per plan year to pay for qualified medical, dental and vision expenses for
yourself and eligible family members. Funds in this account can be used to cover all eligible expenses on your tax dependents even if they are not enrolled under your health care plan. Eligible reimbursable expenses include medical and dental plan deductibles and copays (if applicable), orthodontia expenses not covered by your dental plan, prescription drugs, prescription eyeglasses, and contact lenses, Lasik eye surgery, and much more. A Healthcare FSA is available to anyone enrolled in one of the PPO medical plans.
Limited Purpose FSA - If you are enrolled in the HDHP/HSA plan and open a health savings account, you are eligible to enroll in the Limited Purpose FSA, which allows you to pay for eligible out-of-pocket dental and vision expenses. You are able to put up to $3,050* into your Limited Purpose FSA on a pre-tax basis for 2023.
FSAs do have a use-it-or-lose-it provision, so be conservative when electing how much to contribute. You are eligible to rollover up to $610 to the next year - rollover applies to Health Care FSA and Limited Purpose FSA only.
Dependent Care FSA - You may contribute up to $5,000* per plan year to pay for qualified eligible dependent care expenses. Qualifying dependents must be under the age of 13, share the same residence with you, or be a spouse, qualifying child or relative who is physically or mentally unable to care for him/herself and has income less than the Federal exemption amount. Eligible expenses include: Before/after school care, custodial care for dependent adults, licensed day care centers, nursery schools or pre-schools, and much more. Funds in this account are saved on a tax-free basis.

Basic Life and AD&D Insurance
Basic Life Insurance helps ease your loved ones’ financial burden. Your designated beneficiary will receive a benefit if you pass away from a covered accident or illness. In addition, Accidental Death and Dismemberment (AD&D) provides a benefit to your beneficiary if you pass on or become dismembered due to a specifically covered accident. Always make sure your beneficiaries are updated.
Quotient Sciences provides coverage at two times your annual base salary, up to $500,000. The cost of the benefit is 100% paid for by the company.
Voluntary Life and AD&D Insurance
You may also choose to purchase Supplemental Life Insurance coverage in addition to the company-paid benefit. You pay the total cost of this benefit through convenient payroll deduction.
You must choose Employee Supplemental Life Insurance in order to purchase spouse and child life. Your age and the amount of insurance you elect determines the premium you’ll pay. Costs will go up as you age. Spouse/Domestic Partner benefit terminates when they reach age 70.

Short-Term Disability (STD)
STD covers employees who become disabled as a result of an injury or sickness that is not work related. It replaces a part of the income the employee would have earned had they been able to continue working.
With the STD plan, you are eligible for a weekly payment up to 60 percent of your base earnings, not to exceed $1,000 per week, for up to 26 weeks. Coverage begins on the 1st day of continuous injury or the 8th day of continuous illness. The cost of the benefit is 100% paid for by the company.
Long-Term Disability (LTD)
Keep getting a check when you’re hurt or sick. You always have bills to pay, even when you can’t get to work due to injury, illness, or surgery. Long-term disability insurance helps you make ends meet during this difficult time.
If you become totally disabled, the LTD plan will pay 50% of your base monthly earnings up to a maximum benefit of $2,000 per month. Coverage begins on the 181st day or continuous injury or illness. The cost of the benefit is 100% paid for by the company.
