Medical ▼

Administered through Aetna Signature Administrators

Employees and dependents choose their physicians, healthcare providers, and dentists from contracting providers offering quality care along with substantial discounts. By receiving your health care from providers who contract with our PPO network, you will receive the highest level of benefits the plan offers.

It’s important to keep in mind that the list of contracting providers can change. You should always verify that the provider is contracting with the Aetna Signature Administrators network before each medical service to ensure you receive in-network benefits.

Who is Aetna Signature Administrators?
Our medical network is through Aetna Signature Administrators. Members enrolled in either plan are not Aetna members, so it is important when you receive services that you show your ID card and point out the network listed on the front of your ID card. If providers try to verify eligibility with Aetna, they will not locate you in the system. It must be through Aetna Signature Administrators. If your provider has a question and/or needs to verify eligibility, they can call 866-462-9051 (listed on the back of your ID card).

Who is EBMS?
EBMS is our third-party administrator who provides customer service and claims processing for our medical plan. They also perform medical management, supply ID cards, and communicate to members and providers, in writing, including Explanation of Benefits (EOB). If you have any questions as an enrolled member, EBMS is who you need to contact. You can go online to www.miBenefits.ebms.com or call member services listed on the back of your ID card.

Do I need to see a dentist?
A visit to the dentist is about more than just a teeth cleaning. By looking in your mouth, your dentist can tell a lot about your overall health. In fact, he or she may be able to identify early signs of disease, such as diabetes, heart disease, kidney disease, and even some forms of cancer, before you even notice symptoms.

Disability ▼

Short-Term Disability
Administered by New York Life

Disability insurance is coverage that provides you with income protection should you lose time on the job due to an injury or illness. The risk of disability is greater than most employees realize. When you become disabled and lose time at work, your source of income is eliminated. Nearly one-third of employees will miss more than one month of pay due to injury or illness. In addition to lost income, you are most likely experiencing an increase in medical expenses due to your disabling injury or illness. Traditional medical insurance doesn’t cover every expense related to an injury or illness. Bills and expenses can continue to add up, especially if you have to stop working for a period of time and lose your income.

Benefit Features:

  • Monthly Benefit: 60% of Monthly Benefit base pre-disability
  • Maximum Weekly Benefit: $1,500
  • Waiting Period: 7 Days (Accident of Illness)
  • Maximum Benefit Period: 26 Weeks

Life and AD&D Insurance ▼

Administered by New York Life

Basic Life and AD&D Insurance
Safeguard the most important people in your life. Think about what your loved ones may face after you’re gone. Term life insurance can help them in so many ways, like helping to cover everyday expenses, pay off debt, and protect savings. AD&D provides additional benefits if you die or suffer a covered loss in an accident. We offer this valuable benefit at no cost to you.

Plan Features:

  • Life Benefit: 1 X base annual earnings to a maximum of $300,000
  • AD&D Benefit: For accidental loss of life – same as Life Benefit
  • Age Reduction Schedule: 65% at age 65, 45% at age 70 and 30% at age 75

Voluntary Life Insurance
Voluntary Life Insurance is available in addition to your RV Products-provided Basic Life and AD&D. You may purchase life insurance for your spouse and dependents only if you purchase Voluntary Life Insurance coverage for yourself. Benefits are payable to your beneficiary, so be sure to keep your beneficiary designation up to date.

Plan Features:

  • Life Benefit: 1, 2, or 3 times your basic annual earnings rounded to the next higher $1,000. The maximum for any employee is the lesser of 3 times your annual salary or $200,000
  • Guarantee Issue: Up to $100,000 for under the age of 60
  • Age Reduction Schedule: 67% at age 70 and 50% at age 75
  • Evidence of Insurability (EOI): Not required for amounts up to Guarantee Issue if application is received during initial offering – EOI will be required for late applicants

Contact Human Resources for specific benefit details and/or to update your beneficiary.
The beneficiary designations for your retirement savings, life insurance and other financial accounts take precedence over your will. If you want your wishes to be followed after your death, you must keep your beneficiaries updated. If you have not listed anyone as a beneficiary, your assets will follow a default order to determine who will receive it, such as your spouse, your children, your parents and then your siblings. Listing a beneficiary—and keeping it updated—will save time, effort and potential conflict after you’re gone.
