Pharmacy Benefits
To find a pharmacy near you, go to myprime.com and search by your zip code. Follow the instructions below to find in-network pharmacies near you:

  • Choose "Other BCBSIL plans" from the drop-down
  • Click "Continue"
  • Scroll down and click "Find a pharmacy"
  • Once on the next page, choose "Preferred Network" from the drop-down
  • Click "Submit"
  • Enter your zip code
  • All in-network pharmacies will populate. Please note: anything that is not on that list, is out-of-network.

Health Savings Account (HSA) ▼

A Health Savings Account, commonly known as “HSA,” is an individual account you can open, add money to and spend on eligible health care expenses.

The HSA is Unique Because:

  • Money used for eligible expenses is not taxed.
  • Investment earnings are not taxed.
  • Money spent on eligible expenses is not taxed.
  • The money rolls over year to year.
  • You own the account and you control how the money is spent.

Eligible Expenses:

HSA money can be used tax-free for any eligible health care expenses. In general, eligible health care expenses are those that qualify toward the deductibles, copays, and coinsurance with your health insurance.

The money in your HSA must be used for eligible medical, dental, vision, and prescription drug expenses. If you use money for a dental, vision or medical expense that is not covered by the medical plan, it is important to understand that your medical plan deductible will still need to be met if an expense is incurred.

2024 Annual HSA Contribution Limits:

(Add an extra $1,000 if you are age 55 or older)

  • Single: $4,150
  • Single+1/Family: $8,300
  • Add an extra $1,000 if you are age 55 or older

2024 Silver Birch HSA Contributions:

  • Single: $23.08 per pay period ($600 annually)
  • Single+Spouse / Single+Child(ren): $30.77 per pay period ($800 annually)
  • Family: $38.46 per pay period ($1,000 annually)

The Health Savings Account (HSA) is only available in combination with enrollment in an HDHP plan.

Basic Life and AD&D Insurance through BlueCross BlueShield is provided for eligible team members and pays your beneficiary a benefit of $25,000. This benefit is 100% employer-paid.

Voluntary Term Life and AD&D Insurance allows you to purchase additional coverage at your own financial expense to ease your loved ones’ financial burden if something should happen to you. A team member’s maximum benefit election cannot exceed 5x their basic annual earnings or $500,000. A spouse’s maximum election cannot exceed 50% of what the team member takes out on themselves. Child coverage is also available.

The cost of the benefit is 100% paid for by you. Your age and the amount of insurance you elect determines the premium you’ll pay. Costs will go up as you age.

If you are unable to work due to an accident or an illness, you may face a gap between your income and expenses. Short-Term Disability coverage through Allstate pays you a weekly cash benefit directly to help you cover this gap until you’re back on your feet and back to work.

This coverage is available at low affordable rates and pays you a benefit of up to 60% of your weekly earnings, up to $5,000 per week. The plan begins paying these benefits after you have been unable to work for 14 consecutive days due to an illness or off-the-job accident and will pay you up to 3 months.

Universal Life Insurance with Long Term Care through Allstate provides lifelong protection with guaranteed premiums. This plan protects your family with money that can be used any way they choose. It is most often used to pay for mortgage or rent, education for children and grandchildren, retirement, family debt, and final expenses. This coverage also offers you living benefits should you need Long Term Care after becoming chronically ill and confined to a nursing or assisted living facility, or if you need to receive home health or adult day care.

The unique feature of Universal Life Insurance is that you build cash value with a guaranteed tax-deferred interest rate of 3% which may allow your premiums to remain level throughout the life of the contract.

Employee Assistance Program (EAP) service offers caring and professional assistance for a broad range of concerns including stress management, depression and anxiety, relationship or family conflicts, workplace conflicts, legal or financial difficulties, and drug or alcohol abuse. Services are confidential - neither your employer nor co workers have knowledge of your request for help. EAP services are available 24 hours a day, seven days a week for you and your eligible dependents. There is no cost, it’s just there for you when you need it.

Don't Miss Out!

This annual enrollment period is the only opportunity to enroll in benefits until the next open enrollment period, for benefits effective August 1, 2024, unless you experience a qualifying life event and request a change to your benefits within 30 days of the life event.

Medical and Dental

  • For new enrollees, BCBS will mail ID cards to your home within 2 weeks of your coverage start date.
  • To access a digital ID card, request additional printed cards, or to view plan information, access your account with BlueCross BlueShield at www.bcbsil.com.  Under Member Log-In, click Log In To Your Account. 
    • If creating a new account, click Register Now and have your Group # & Subscriber ID # ready.  Contact HR if you need this information.
  • Members can also download the BCBS app. Click here for more information.

Vision
