Flexible Spending Accounts (FSAs) have become a popular vehicle for reducing rising health care costs. By contributing pre-tax dollars into an FSA, you can save an average of 20% on eligible expenses every year.

Health Care Flexible Spending Account
Employees use pre-tax dollars to pay for insurance deductibles, copayments, glasses and contact lenses, orthodontia, over-the-counter medications, and hundreds of other health care-related expenses not covered by their insurance plans. The maximum contribution amount for period 01/01/2024 through 12/31/2024 is $2,850.

Please Note: Under the CARES Act effective March 27, 2020, claims for most over-the-counter medicine, medical supplies and feminine hygiene products can be purchased through an FSA without a prescription. This reverses the prior rule from January 1, 2011 in which a prescription was required. The Act is effective retro back to January 1, 2020.
Why Participate?
Do you purchase glasses or contacts every year? Will you need to pay for dental expenses? Do you need extra money to pay for health expenses? You may be able to save on taxes by contributing to the Health Care FSA. You use your own money – but it’s before tax money so you could pay less in taxes. Estimate carefully. You forfeit funds set aside in the FSA if you do not use them.

Your Dental plan is provided through Delta Dental of Indiana. Find a dentist at www.deltadentalin.com. Your Plan pays $1,500 per person total per Benefit Year on all services, except preventive and diagnostic services.
You are responsible for a $50 Deductible per person total per Benefit Year limited to a maximum Deductible of $150 per family per Benefit Year. The Deductible does not apply to oral exams, preventive services, X-rays, brush biopsy, sealants, and orthodontic services.

Life and accident insurance gives you financial protection by paying a benefit if you die or are seriously injured in an accident.
Basic Life Insurance
As an important part of your personal financial planning, TMC provides you with employee life and accidental death and dismemberment (AD&D) insurance coverage through Mutual of Omaha at no cost to you. You automatically receive life and AD&D coverage on the first of the month following 60 days of employment in the amount of $15,000. This benefit does not have cash value.
Voluntary Life Insurance
You have the opportunity to purchase additional life insurance coverage for yourself and for your spouse. If you are a new hire you may enroll for coverage up to the Guarantee Issue Amount. If you enroll after your initial new hire period you may be required to complete Evidence of Insurability (EOI). If EOI is not completed or accepted by Mutual of Omaha you will not be covered.
Consider costs such as funeral expenses, legal expenses, and general living expenses for your surviving family members when determining an appropriate amount of additional coverage. You must elect coverage on yourself if you wish to cover your spouse.
If you are under age 65 on your effective date of insurance, the amounts of your Basic Life, Basic Accidental Death and Dismemberment and Supplemental Life Insurance on and after age 65 will be reduced by the appropriate percentage from the following table to the amount of Your insurance in effect on the day before Your 65th birthday:

The Hospital Indemnity insurance plan pays benefits when you have a planned, or unplanned hospital stay for an illness, injury, surgery or having a baby. The plan pays a lump-sum benefit for admission and a daily benefit for a covered hospital stay. You can use the benefits to help pay out-of-pocket medical costs or personal expenses.

Short Term Disability
Short Term Disability (STD) insurance plan provides a stable income source to carry you and your family through a temporary disability. In the event of a non‐work related accident or any non‐work related debilitating event, a portion of your salary will be continued for a period of time.
Long Term Disability
Long Term Disability (LTD) pays a portion of your salary in the event you are permanently disabled and unable to work.

When am I eligible for the Plan?
You are able to participate in the Plan if you complete 3 months of service, are at least 18 years old and you are not covered by a collective bargaining agreement (unless the agreement specifically provides for you to be covered by the plan) or are a HCE other than a TMC employee with the title of Group Leader. Once you satisfy these requirements you will become eligible to participate in the Plan on the first day of the following month.
