To use the self-service enrollment site, you must have your Member ID and your confidential Personal Identification Number (PIN). Your PIN is the last four digits of your Member ID plus the four digit year of your birth.
To ensure you understand your benefit options, we have partnered with EOI Service Company. EOI's professional benefit counselors will be available by appointment to help guide you through the enrollment process and help you enroll in the benefits best suited for you and your family.
