  • You will be asked which network you belong to please state ‘Superior Vision’ as this is the network we are leveraging through MetLife.
  • From there, you will be connected to a call center rep and you’ll be asked to verify some basic information- first/last name, DOB, and home address.
  • From there, your coverage will be verified. 
Even if you don’t wear glasses, regular visits to your eye doctor are important to your overall health. That’s because a routine eye exam can often detect early signs of serious health conditions like diabetes.  A group vision plan makes it easier to get the care you need and lower your costs. 

The MetLife Superior Vision Plan features:

  • Low to no copays for in-network services and materials, including contact lens fittings and evaluations
  • The freedom to visit any licensed eye care professional or choose from thousands of participating ophthalmologists, optometrists, and opticians/optical retailers. Your out-of-pocket costs will usually be lower when you visit an in-network provider.

Flexible Spending Account (FSA) ▼

The Flexible Spending Account allows employees to take advantage of the provisions in Section 125 of the Internal Revenue Code. This Code section provides that payroll deductions for health insurance, dental insurance, life insurance, and child care will be taken on a before-tax basis. This plan must always comply with the latest requirements of the IRS code and therefore changes may occur without notice.

Healthcare Flexible Spending Account: 

During open enrollment, you set amounts for the upcoming year (minimum $100 – maximum $3,300) based on an estimated amount of medical expenses for the upcoming year. The amount is divided by the number of pay periods in the year and then redirected from each paycheck – before taxes – into your account. The Flexible Spending Account is provided by Inspira. You receive a debit card for most expenses, or you can submit your receipts and a claim form as found on the Forms section of the WMC intranet. Either way, the account reimburses you in tax-free money.

You may incur expenses through the end of the plan year (ex. 2025 Plan Year ends 12/31/2025).  Unused funds at the end of the plan year, up to $660, will rollover to the next plan year and continue to be available to you for expenses occurred in the next plan year.  Any unused funds above the $660 rollover amount will be forfeited.

Dependent Care Flexible Spending Account:

This plan works much like the Flexible Spending Account mentioned above. At the beginning of each year, you decide the amount needed to cover dependent care costs over the course of the year. When you incur eligible dependent care expenses from any allowable provider, the account reimburses you in tax-free money. This covers most of the expenses related to the care of an eligible dependent so that you can work full time. If you are married, both you and your spouse must work full time, or your spouse must be disabled or a full-time student, to qualify for this account. The maximum contribution annually is $5,000.

You may incur and submit expenses up to 90 days after the plan year ends (ex. 2025 Plan Year ends 12/31/2025, incur/submit deadline 3/31/2026), this is called a grace period.  Any unused funds after the grace period will be forfeited.

    • Click Register on the top right of the page.
    • Click the Accident link under Employee Benefits.
    • Enter your information.
    • Validate your identity and create a username and password. Click Log in Now.
    • Enter your username and password and create your security question.
  2. Download a claim form
    • Click on My Claims.
    • Click the Download Claim Form.
    • Select the Accident claim form.
  3. Complete the form
    • Fill out all information and save the form to your computer.
    • Click Cancel to close the Download Form window.
  4. Submit the form
    • Click on Submit a claim on the Home page.
    • In the pop-up window, click Browse, select the completed claim form and hit Open.
    • Add a description for the document and any additional comments, if needed.
    • Click Submit Claim. You will get a message that the upload was successful.

Paid Time Off ▼

Paid Time Off is accrued time based on years of service. Paid Time Off is a consolidation of holiday time, vacation time and sick leave. PTO will also be used for bereavement days that are not covered by the Bereavement Policy.

Paid Time Off is accrued starting with the first day of employment and may be taken after satisfactory completion of the ninety-day provisional period. The provisional period will also apply to employees changing from PRN to benefit eligible.

In the month prior to the employee’s anniversary date, PTO balances will be reduced to the amount of PTO earned annually. PTO in excess of the annual PTO earnings will be paid to the employee at 75% of base pay and be reflected on the next regular paycheck.

Qualifying Life Event ▼

Qualifying life events include:

  • Change in legal marital status, including marriage, divorce, death of a spouse, legal separation, or annulment
  • Change in number of children, including birth, adoption, placement of adoption, or death of a child
  • Change in employment status of employee, spouse, or dependent, including termination or commencement of employment, commencement or return from unpaid leave of absence
  • Change in dependent eligibility status, including attainment of age, student status, or any similar circumstance

The change to your benefit elections must be consistent with the qualifying life event. You have 31 days from the qualifying life event to request your enrollment change. In most cases, your election will become effective the day of the qualifying event. Otherwise, you must wait until the next annual enrollment period to make a change to your elections.

Worker’s Compensation Insurance ▼

Williamson Health complies with all the laws of the State of Tennessee requiring employers to provide protection for on-the-job injury or job related
illness.

  • If you become injured or ill at work report to your supervisor immediately
  • An accident no matter how minor or serious must be reported
  • An event notification must be completed by the employee

Reporting a Leave ▼

When Do I Report a Claim/Leave?

Your own serious illness, disability, or maternity leave: You may report a claim up to 30 days in advance of a planned disability absence OR as soon as you are aware that you will be disabled due to illness or injury for 14 or more calendar days. Your family member’s serious illness, military leave, or your own intermittent leave: You may report a leave when you will be out of work for more than 3 consecutive days or intermittently to care for an immediate family member suffering a serious illness or to care for a newborn, foster or adopted child.

How to File a Claim

Rest Periods ▼

  • Employees receive 2 – 15 minute rest periods each day
  • 1 – 15 minute rest period granted for those who work less than a complete shift
  • 1 – 30 minute lunch granted for each day

All rest periods and lunch periods should be scheduled with your supervisor or director.

Educational Assistance ▼

It is the policy of the Williamson Health to support the development of a better educated and more highly skilled workforce by providing educational assistance to its employees in accordance with the guidelines established.

The projected degree must fulfill the requirements for a needed, employable position at Williamson Health.

Your donations to this fund help support areas of greatest need within Williamson Health.

With the support from our 2024 STaT program, we were able to fund:

  • Grants for the Larry Hough Employee Relief Fund
  • Gas Cards for breast health patients who are in need that travel from far distances to receive treatment at Williamson Health
  • Vein Finder for PEDS
  • Bladder scanners for all floors of Williamson Medical Center.
  • Academic scholarships to employees and local students furthering their education in healthcare.
  • Items that the Child Life Specialist distributes to our patients in the Monroe Carell Jr. Children’s Hospital Vanderbilt at Williamson Medical Center
  • Swaddles for the babies in the NICU
  • New equipment for Cardiac Rehab
  • New recliners for the Operation Pink Chair Program
  • Williamson County Athletic Training Program
  • Educational materials for new moms unprepared to take their baby home.
  • Quiet Nights- a program to promote restful nights for our patients

If you wish to donate to the Williamson Health Foundation via payroll deduction.

  1. Designate a monthly donation amount during your benefit appointment or in bswift.
  2. To designate the funds to which your donation should be applied, complete and return this form to and return it to Rachel Caudle (rcaudle@williamsonhealth.org) in the Foundation office.
  3. Donations will be deducted from your paycheck for 26 pay periods in the year 2025.
  4. Everyone who participates, regardless of the amount or frequency of gift, will receive a STaT helmet for treats throughout the year ahead!
  5. Anyone who pledges $5 or more per pay period or a one-time gift of $130 will receive a discount code for the Williamson Health Online Store.
