Illinois and Non-California Team Members
Team members who reside in a state other than California may choose to participate in the Cigna medical plan. This plan features an employer-funded Health Reimbursement Arrangement (HRA) which you can use to pay for your qualifying out-of-pocket medical expenses. The plan will reimburse you 75% of your eligible medical expenses up to a maximum of $2,200 per family or individual. Up to $500 of unused funds can be carried forward to the next year. Teladoc Health ▼

Team members enrolled in the Cigna medical plan and team members not enrolled in a medical plan through EOI will have access to Teladoc Health at no cost to them. You must have medical insurance to use this service.

Through Call a Doctor Plus (CADR+), Teladoc Health provides 24/7/365 access to a doctor via phone, computer, or mobile app. The program doesn’t have any deductibles or co-pays, meaning that you have quick and easy access to a doctor. All physicians are board-certified and will conduct the consultations over the phone or through video, just like an in-person visit. If a prescription is required, the doctor will send it to your pharmacy of choice.

This benefit is paid for by EOI and is offered to you and your dependents at no cost. You will receive a welcome email with instructions on how to activate your membership. You will also receive an invitation to join an orientation webinar as well as a benefit card.

Kaiser Participants
If you are enrolled in Kaiser you have access to telemedicine appointments via Kaiser.

Flexible Spending Account (FSA) ▼

Flexible Spending Accounts (FSA) allow you to pay for qualified expenses using pre-tax dollars. The money deposited into your spending account is deducted from your paycheck before taxes are withheld, which lowers your taxable income and increases your spending power. Benefits-eligible employees may only register for an FSA in December or upon new hire.

A Medical Flexible Spending Account is available to you to pay for qualified expenses. The annual maximum contribution is $3,200.

The Dependent Care Spending Account helps you pay for daycare expenses for a dependent under the age of 13 or for care of a disabled spouse or parent while you work. Eligible expenses include things like payments to a licensed daycare provider or nursery school, before and after school care, a summer day camp program, or elder care. The annual maximum contribution is $5,000 per family or $2,500 for married employees filing a separate tax return.

Use it or lose it! FSA expenses incurred during the calendar year must be submitted for reimbursement by March 15th of the following year or they will be forfeited.

Retirement Planning ▼

Our 401(k) plan offers you a way to save for your retirement. This plan provides significant tax benefits during your working years and enables you to build up an account for future use. The plan requires the employee to designate a portion of his/her paycheck to be contributed by payroll deduction. This contribution is withheld each pay period and is deducted before federal and state income taxes are withheld.
 
If you contribute to the plan, you will be eligible for a matching contribution from EOI. For every dollar you contribute, EOI provides a matching contribution of 50%, up to a maximum of $200 per year. To receive this match, you must be employed on December 31st of that particular year. All employee contributions and employer match contributions are 100% vested immediately and cannot be lost regardless of when you leave the company. Participants have a choice of investment for their funds. You may change your election or investment funds at any time. More information is available in the 401(k) packet you will receive upon eligibility. You will be eligible to participate in the plan when you have attained age 21 and completed one year of service. Your entry date will be the first day of the plan year quarter coinciding with, or next following, the date you satisfy the eligibility requirements.

Profit Sharing
 
In addition to the matching contribution, EOI may periodically make profit sharing contributions based upon the financial success of the Company. You must be employed at EOI on December 31st and have worked more than 1,000 hours in the calendar year to receive profit sharing contributions. The following vesting schedule is
provided to determine when an employee has a non-forfeitable claim upon the profit sharing contribution.

Life and AD&D Insurance ▼

Basic Term Life Insurance and AD&D is paid for by EOI and provided at no cost to you in the amount of $40,000.

Supplemental Term Life Coverage is available in addition to your EOI-provided basic life insurance. You may also purchase life insurance for your dependents if you purchase additional coverage for yourself. An Evidence of Insurability Form will be needed if you choose to elect an amount greater than the guaranteed issue amount of $100,000 or if you increase your coverage after your initial enrollment period; the E of I form can be found below. Please submit your completed E of I form to EOI Benefits Team at eoibenefits@eoiservice.com.

LifeTime Benefit Term Insurance with Long Term Care ▼

LifeTime Benefit Term Insurance with Long Term Care Coverage is available in addition to your EOI-provided basic life insurance. LifeTime Benefit Term is permanent term insurance that can last a lifetime, with premiums guaranteed for life and coverage for qualified long term care expenses like nursing home, assisted living, or home care. The plan is portable and coverage is also available for your spouse and children.

This plan includes guarantee issue up to $75,000 for employees (actively at work) ages 19-70. You have the option to purchase up to $150,000 in coverage on a conditional guarantee issue basis. A short medical questionnaire must be completed for additional coverage.

Disability Protection▼

Short Term Disability Insurance (STD) through MetLife provides income if you become disabled due to an injury or illness and are unable to work. You can receive a benefit of 60% of your weekly base pay, up to $1,500 per week, offset by other income benefits such as workers’ compensation and California State Disability. Benefits begin after 14 days and last for a maximum of 13 weeks. There is a limited benefit for pre-existing conditions; maternity is covered as any other disability. The policy is provided at no cost to you and is paid for by EOI.
 
How to File a Claim
EOI HR team will complete section 1 of the STD form.  You will complete section 2.  and your doctor will fill out section 3.  You must sign in TWO places on page 3 and one place on page 5.  EOI HR team and doctor sign on page 5.  Missing signatures is the most common reason for claims being delayed.  Nothing can be processed until the forms are signed.  The completed forms can be sent via traditional mail, fax, or emailed to mtpmetlife@metlife.com.  The fax number and mailing address are on the top right-hand side of the first page of the form, by the MetLife logo.  If submitting via email, the ER would need to include the same information in the email that they would for faxing, a completed employee statement, and a completed employer statement.  They also need to include in the subject line:  New Claim – Customer Number – Employee Name.  Once the claim is initiated and assigned a Claim Specialist (CS), the CS will reach out you directly for any more needed information.

Long Term Disability Insurance (LTD) provides a source of income should you be unable to work for an extended period of time. The policy pays a benefit of up to 60% of your base monthly pay, up to $10,000 per month, offset by other income benefits such as workers’ compensation and California State Disability. Benefits begin after 90 days. The policy is provided at no cost to you and is paid for by EOI.
 
How to File a Claim

EOI Cares Volunteer Program ▼

Team Members can volunteer both virtually or in person in their community. Time off for face to face volunteering must be requested via Paylocity the same as it would be requested for sick or vacation time. All are welcome to volunteer, however face to face volunteering is only applicable to benefit eligible team members, as this is paid time off.

Virtual Volunteer Opportunities:

Volunteer Time Off (VTO)

EOI is offering 8 hours of face-to-face community volunteering. 8 hours can be taken all at once to volunteer for a full day, or in hourly increments. For time keeping purposes the minimum amount of time that can be taken is 1 hour at a time. Volunteer time off is paid and needs to be approved by your supervisor before it can be used. See below for details and requirements:

  • You will request the time off the same way you currently request sick and vacation time. This way we can ensure that this doesn’t interfere with the needs of the business.
  • Use the Volunteer Time Off (VTO) code in Paylocity on your timecard.

You must also provide proof of volunteering. There are two ways to provide proof:

  • Click here to download the volunteer form and take it with you to obtain a signature from the organization you are volunteering with. The form needs to be turned in to HR after as proof of volunteering. (You can disregard using our form if the organization you volunteer with has their own document or a confirmation of your request and acceptance for volunteering with them.)   • Take a selfie at the volunteer event and submit to HR at mbrown@eoiservice.com. In your email please include:

Paid Time Off ▼

New employees begin to accrue paid time off the first day of the month after the completion of 60 days of service. Paid time off accrues on a per paycheck basis; however, the time cannot be taken off with pay until it is earned. The accrual of paid time off is based upon the years of continuous service, as of January 1st, in accordance with the following schedule:

Holidays

EOI offers eight paid holidays:

  • Memorial Day: Monday, May 27th
  • Independence Day: Thursday, July 4th
  • Labor Day: Monday, September 2nd
  • Thanksgiving Holiday: Thursday, November 28th
  • Thanksgiving Holiday: Friday, November 29th
  • Christmas Eve Holiday (starting at noon): Tuesday, December 24th

Sick Days

In addition to paid time off and holidays, EOI also offers six paid sick days per year. Your sick days do not carry over.

Volunteer Time Off (VTO)

The 8 hours provided for VTO can be used all at once or in as little as 1 hour increments. The time will not roll over, it will be use it or lose it from year to year. It is set to expire on 12/30 of each year and refresh on 1/1 of each year. Please contact Marquita Brown, HR Manager if you have questions about activities you want to volunteer for. Some examples would be, a church, your child's school or any neighborhood organization or established volunteer organization like volunteers of America or habitat for humanity to name a few.

Don't Miss Out!

This enrollment period is the only opportunity to enroll in benefits until the next annual enrollment period unless you experience a qualifying life event.

What is a qualifying life event?

Certain coverages allow limited changes to your benefit elections during the year. These benefits include the Medical, Dental, Vision, Life, and Disability. For these benefits, you may only make changes to your elections during the year if you have a qualifying life event.

Qualifying life events include:

  • Change in legal marital status, including marriage, divorce, death of a spouse, legal separation, or annulment
  • Change in number of children, including birth, adoption, placement of adoption, or death of a child
  • Change in employment status of employee, spouse, or dependent, including termination or commencement of employment, commencement or return from unpaid leave of absence
  • Change in dependent eligibility status, including attainment of age, student status, or any similar circumstance

The change to your benefit elections must be consistent with the qualifying life event. You have 30 days from the qualifying life event to request your enrollment change.

Eligibility
The benefits outlined above are available for full-time employees on the first day of the month following the completion of 60 days of employment, unless otherwise stated. For example, an employee hired on April 4 would be eligible for benefits on July 1. When you first become eligible, or during the open enrollment period, you are able to select insurance for yourself and your dependents.

Employees who have a domestic partner shall be provided services and benefits on the same basis provided to legal spouses unless restricted by law. Subject to the eligibility provisions of the benefits plans, an employee has the opportunity to cover a same-sex or opposite-sex domestic partner under his or her benefit plan under the same terms and with the same privileges and restrictions that apply to other eligible dependents for these plans and to the extent permissible under federal and state law.
