Terms You Need to Know ▼

Health Insurance can be confusing.
Here’s a list of healthcare terms you might need to know to help you choose a plan and then use that plan effectively.

Open Enrollment: The period of time each year when a health insurance plan allows members to enroll or change their plan.

Premium: The amount you or your employer pays each month in exchange for your health insurance.

Copay: The fixed amount you pay up front when you receive medical service. For example, some plans require a $30 copay for a visit to a Primary Care Physician and a $60 copay for a visit to a Specialist.

Deductible: The amount that you must pay for medical services before your health insurance plan begins to contribute payment. After reaching this amount, the insurance covers their percentage of your services. The lower the deductible, the sooner insurance starts to pay.

Coinsurance: This is the percentage that you pay of your medical bills, after fulfilling your deductible amount. For example, if the coinsurance percentage is 80% and the cost of your x-ray was $1,000, the insurance would cover $800 of the bill, and you pay the remaining $200. This charge is in addition to your copay. Coinsurance is paid until the out-of-pocket maximum is reached.

Out-of-Pocket Maximum: This is the maximum amount you will have to pay for your medical expenses for the entire year, as long as the doctors and services are covered and in-network. After this amount is reached, insurance pays 100% of covered, in-network medical bills and pharmacy expenses.

In-Network: These are providers the insurance company has selected and agreed upon contracted rates for medical and pharmaceutical expenses. Your costs are lower with in-network providers and some plans only pay when you stay in-network for your care.

Out-of-Network: These are providers that have NOT agreed upon the insurance contracted rates for medical and pharmaceutical expenses. You may pay more when you receive out-of-network care and some plans do not cover these providers or services at all.

Provider: Any person or institution that provides medical care. Examples of providers include doctors, nurses, hospitals, and clinics.

Claim: A request by a plan member or healthcare provider that the insurance company pays for its share of a medical service. Your insurance plan receives a claim every time you receive medical care.

Pre-Authorization: When your insurance requires a pre-approval for a service or drug to ensure that the service or drug is medically necessary and that your insurance will help pay for it.

Health Insurance Through ICHRA

WHAT IS ICHRA?

An Individual Coverage Health Reimbursement Arrangement (ICHRA) is a modern way for you to get health coverage. Instead of being limited to a one-size-fits-all group plan, ICHRA gives you more flexibility and control over your healthcare by letting you choose the individual plan that best suits your needs. Your employer will contribute monthly towards your health insurance premiums.

  1. Log into The Big Plan Platform: You’ll receive an email with a link to log into The Big Plan (TBP) platform. Once logged in, you can review your employer’s ICHRA contribution.
Important: This is not the cost of your health insurance; it’s the amount your employer is contributing toward your plan.
  2. Complete the Health Questionnaire: Answer a few simple health-related questions (you can skip this if you’d like) to help the system recommend the best plan for your needs.
  3. Review Your Plan Options: You’ll be shown three main plan options, and the prices displayed will already factor in your employer's ICHRA contribution:
    • Best Match: Based on your questionnaire answers.
    • Lowest Premium: Usually a high-deductible plan with lower monthly costs.
    • Cheapest $0 Deductible: For those who want no upfront costs when receiving care.
You can explore all available plans if you'd like.
Important: Some plans may be suggested even if your doctor is out-of-network. The platform will indicate which doctors are in-network; however, it is important to call the provider to make sure they take the plan.
  4. Complete Your Application: Fill out the application on the TBP platform. Be sure to have the following information handy for all covered members:
    • Date of Birth
    • Social Security Number
    • Any other required personal details
    • Any family members information (DOB, SSN etc.)
  5. Application Review: Once submitted, The Big Plan team will review your application within 2–3 business days before sending it to the insurance carrier.
  6. Submission to Carrier: Once The Big Plan submits your application to the insurance carrier, there may occasionally be questions or additional information needed from you. If this happens, we will contact you immediately.
Important: Please respond promptly to any requests to ensure your application is processed without delays. Keep an eye on your email for any communication from us.

Payments
The Big Plan (TBP) will work with your employer to cover the entire premium, even if your employer’s ICHRA contribution is less than the full price of your selected plan. The difference between your employer’s contribution and the total premium will be deducted from your paycheck.

Important: The plan you are enrolled in is an individual plan. You may receive a bill in the mail reflecting the full premium. Do not pay this bill or cancel your policy. If you receive a delinquent notice or a notice of cancellation, contact us immediately at help@thebigplan.com. Once your plan is active, you’ll receive an email confirmation.

Accident ▼

Accidents can happen anytime—at work, during your commute, while playing sports, or even doing something as simple as walking down the stairs. And when they do, the unexpected costs can add up fast. Accident Insurance through MetLife helps fill the financial gaps left by your medical plan by paying cash benefits directly to you, regardless of other insurance you may have.

This 24/7 coverage applies whether the injury happens on or off the job and includes a wide range of scenarios. Benefits are paid for:

  • Broken bones and dislocations

  • Concussions, burns, and eye injuries

  • Emergency room visits and ambulance rides

  • Follow-up doctor visits and physical therapy

  • Hospital stays (regular or ICU)

  • Surgery, prosthetics, and medical appliances

  • Accidental death, dismemberment, or paralysis

There's also a $50 annual health screening benefit that rewards you for completing preventive screenings like physicals, mammograms, or cholesterol checks. Plus, if the accident occurs while participating in an organized sports activity, benefit payouts may increase by 25%, helping cover even more of your out-of-pocket expenses.

You can use the cash however you need—to help with medical bills, travel costs, child care, or even everyday household expenses while you recover.

Critical Illness ▼

When a serious illness strikes, it can bring more than just a health crisis—it can also bring financial uncertainty. MetLife Critical Illness Insurance offers peace of mind by paying a lump-sum benefit directly to you if you're diagnosed with a covered condition. This money is yours to use however you need—whether that’s medical bills, travel to specialists, groceries, or rent.

Coverage includes a wide range of conditions:

  • Cancer (invasive and non-invasive), heart attack, stroke, and major organ transplant

  • Kidney failure, ALS, Alzheimer’s, and Parkinson’s

  • Infectious diseases (like COVID-19, encephalitis, or tuberculosis) that result in hospitalization

  • Childhood illnesses including type 1 diabetes, cerebral palsy, and spina bifida

If you experience a second occurrence of a previously covered illness, many conditions are also eligible for a recurrence benefit—meaning you could receive a second lump-sum payment for the same diagnosis after a designated suspension period.

You’ll also receive a $50 health screening benefit each year just for completing preventive care like a wellness exam or certain lab tests. Coverage is available for you and your eligible family members, and it’s portable—so you can take it with you if you leave your job.
