Eligibility

Benefits are available to part-time and full-time Caregivers and their dependents who meet specific eligibility requirements. Registry caregivers are eligible for designated medical plan options. The plans generally allow coverage for a Caregiver’s legal spouse, civil union partner and/or child(ren), including biological, adopted, or stepchildren, covered from birth to the end of the month they turn age 26; to age 30 for honorably discharged veterans.  When enrolling dependents, you will be required to submit proof of dependent eligibility, including the Social Security Numbers and dates of birth for your eligible dependents, and/or anyone whom you wish to name as a life insurance beneficiary. New Hire or Newly Eligible Enrollment

Newly hired or newly eligible caregivers must enroll in benefits during your first 30 days of being hired into an eligible position. Benefits coverage starts on the 1st of the month after 30 days of new eligibility or new hire employment.
 
Once enrolled, changes to your benefits during the year are limited per IRS regulations. If you have a qualified life event, you may request a change within 30 days of the change. See Benefit and Eligibility Changes or your benefit guide for a list of qualified events.

Using Bswift the Enrollment System

Eligible caregivers may use the online enrollment system to enroll for the first time, check on their enrollments, and make annual enrollment choices.  

How to Prepare for Open Enrollment:

  • Visit SinaiBenefits.bswift.com and test your bswift password. If it doesn’t work or you forgot it, then reset your password before you get locked out on 3 failed attempts.
  •  If needed, use the following to reset your password:
    • User name: Date of birth in "mmddyy" format followed by the last four digits of your Social Security Number
      Example: Date of birth 06/13/80, Social Security Number 123-45-6789 = 0613806789
    • Password: Date of birth in "mmddyyyy" format
      Example: Date of birth 06/13/1980 = 06131980
  • Confirm that your home address and mobile phone number are accurate in bswift.
  • Be prepared to provide the Social Security Numbers and dates of birth for your eligible dependents (i.e., spouse and children) whom you wish to enroll in benefits for the upcoming calendar year, and/or anyone whom you wish to name as a life insurance beneficiary.

Disability income protection insurance provides a benefit when you are unable to work due to a serious covered injury or sickness. Sinai Health System provides Long-Term Disability protection for all non-union Caregivers. The Base Plan pays a benefit following 180 days of disability. The monthly benefit is an amount equal to 40% of your covered earnings and continues for a two-year benefit period.

Critical Illness Insurance

No one can be completely prepared when a critical illness strikes, but if you or a loved one is diagnosed with a covered condition such as cancer, stroke, or heart attack, Voluntary Critical Illness Insurance provides a lump-sum cash benefit to help pay for out-of-pocket medical expenses or any other bills that need attention, including rent or groceries. And every year that you and a covered spouse complete a qualified health screening, you are eligible for a $50 Health Screening Benefit. This Health Screening Benefit includes COVID-19 testing, so if you test for COVID-19, you will receive the $50 benefit.
