Plan Changes ▼

  • Traditional Medical Plan Ending: The Traditional medical plan will no longer be available. If you are currently enrolled in this plan and wish to continue medical insurance, you must enroll in Symbria’s Select or Consumer Choice plan.
  • Medical Out-of-Pocket Max Changes: Slight change to the out-of-pocket maximum for both the Select and Consumer Choice Medical Plans. Individual $7,500 and Family $15,000.
  • Full-purpose Health Care FSA will no longer be available due to IRS rules. Limited Purpose FSA will still be available to anyone enrolling in our high-deductible health plan options. Limited purpose FSA can be used for dental and vision expenses only. Limited purpose FSA does not reimburse for medical or Rx expenses.
  • Post COVID-19 Public Health Emergency (PHE): The Nation’s Public Health Emergency status will end on May 11, 2023. There are changes to your health plan coverage for COVID-19 diagnosis and treatment that will take effect. Prior to the end of the PHE, the federal government was funding most COVID-19 related healthcare at 100%. However, that funding will end as of May 11, 2023.
  • The following Post COVID-19 PHE changes will take effect after May 11, 2023:
    • COVID-19 health services will be treated as any other illness. Your applicable deductible, coinsurance or copays will apply, depending upon where you seek services. This applies to physician visits (including Urgent Care or ER), testing and lab services, and prescription drugs.
    • COVID-19 OTC tests will no longer be covered or reimbursed by the health plan. OTC tests are eligible for reimbursement under Symbria’s HSA.
    • COVID-19 Immunizations will now be covered under your preventive care benefit (like a flu shot or other covered immunization).
  • New Virtual Care Provider: Teladoc will replace MDLive for your virtual care services.

New Virtual Care Provider ▼

Teladoc will replace MDLive as our virtual care provider. With Teladoc, BCBSIL members get access to:

  • General Medical: Diagnosis and treatment by board-certified doctors for cold/flu, allergies, sinus infections and more, with prescriptions (if necessary) sent to your local pharmacy.
  • Behavioral Health: Support for depression, anxiety, stress and more from a therapist or psychiatrist available by appointment 7 days a week.
  • Dermatology: Upload photos through the secure portal and get custom treatment plans for skin issues such as eczema, psoriasis, acne, rosacea, skin infections and more.

Flexible Spending Account (FSA) ▼

Our Flexible Spending Account (FSA) options are administered by EBC. These accounts allow you to save money on a pre-tax basis to pay for qualified dental and vision expenses and/or dependent care expenses you may incur throughout the year.

Limited Purpose FSA (LPFSA) - The Limited Purpose FSA is only available to employees enrolling in one of our high-deductible health plan options.  A Limited Purpose FSA allows you to pay for eligible out-of-pocket dental and vision expenses only.  Limited Purpose FSA does not reimburse for medical or Rx expenses. Your HSA election should be maximized first, before contributing to the Limited Purpose FSA. You are able to put up to $3,050 into your Limited Purpose FSA.

FSA Minimums and Carryover

  • There is a minimum annual contribution of $260 ($10 per paycheck for the full plan year). The minimum applies to both the Limited Purpose FSA and Dependent Care FSA.
  • Employees currently participating in the FSA that do not elect new LPFSA contributions during open enrollment are not eligible to roll over any unused funds. No rollover will apply if you do not re-enroll.  
  • You are eligible to roll over a maximum of $570 if you elect to contribute in the new plan year. The minimum carryover amount is $50. There is no rollover for Dependent Care FSA.
  • You have 90 days past the end of the plan year (Sept 30) to submit receipts for reimbursement

Dependent Care FSA (DCFSA) - You may contribute up to $5,000 per plan year to pay for qualified eligible dependent care expenses. Funds in this account are saved on a tax-free basis. No rollover applies. 

EBC Information

  • Registering Your EBC Account - To register your account, go to www.ebcflex.com and choose "Participants" from the "Log In" drop-down menu. On the log in page, click "Register". By registering yourself, you’re able to submit claims, view transactions and check the balance of your account(s).
  • FSA Debit Card - Upon enrolling in the FSA, you’ll receive a debit card to use on your qualified expenses. For a list of what’s qualified, see the below Additional Resources.
  • Substantiation Options - Claims may need to be substantiated in order to be paid. EBC may do this randomly to make sure FSA funds are only being used for qualified expenses. Unsubstantiated claims may result in the deactivation of the card. Please review ways in which to substantiate in the carrier information section in your benefit guide.

Symbria offers dental coverage through Blue Cross Blue Shield.

How the DPPO Plan Works
A DPPO allows the flexibility to select any dentist in-network or out-of-network. The plan pays a higher level of benefit when you go to a dentist who is contracted with the BlueCare Dental PPO Network. If you go to an out-of-network dentist, your benefits will be based on 80% of reasonable and customary (R&C) charges for a particular service in your area. If the dentist charges more than 80% of R&C, you will be expected to pay the difference.

Blue Cross BlueShield PPO dentists have agreed to charge BCBS plan members significantly reduced rates. Blue Cross Blue Shield PPO has a national network of more than 139,000 dentists. That’s 40% more providers than most large dental networks.

Out-of-Network dentists can charge whatever they like for their services. The plan will only pay benefits up to Blue Cross Blue Shield’s approved fee. If your non-Blue Cross Blue Shield dentist charges you more than the approved fee, you will have to pay the difference. Also, you may have to pay the full bill upfront. You will then submit a claim to Blue Cross Blue Shield to be reimbursed for the covered portion of the charges.

Vision ▼

Symbria offers vision coverage through EyeMed. Vision insurance helps offset the costs of routine eye exams and also helps pay for vision correction eyewear, like eyeglasses and contacts, that may be prescribed by an eye-care provider. By accessing in-network vision providers, you're able to reap the benefit of true vision insurance coverage. You are now able to either (1) utilize the frame allowance and lens copay in the same calendar year or (2) utilize the contact lens allowance and frame allowance (including the 20% discount on lenses) in the same rolling 12 months. You are not able to get contacts and utilize the lens copay for your frames in the same rolling 12 months. 

Out-of-network providers merely offer you an allowance towards your vision services. If you go out-of-network for your services, you will pay the entire cost up front and then file a claim with EyeMed to get reimbursed for your services up to the allowance listed below. 

Eye-care providers include many independent optical shops and national chains. Some examples include LensCrafters, Pearle Vision, Target Optical and many others.

401 (k) ▼

Symbria offers a 401(k) Program through Securian. The 401(k) Plan is an employer-sponsored retirement plan that allows employees to save and invest for the purpose of building savings for retirement. Saving through a 401(k) Plan is an easy way to set aside money for your future. Some things to consider: 

Contributions to your account: 

  • You can make pre-tax and/or Roth (after-tax) contributions up to the IRS calendar year dollar limit ($22,500)
  • If you are over the age of 50, you may make an additional $7,500 catch-up contribution during the calendar year
  • Newly eligible participants can begin contributing on your entry date as specified by the plan
  • Existing plan participants, or those who have met their entry date, can make changes at any time
  • You can rollover money into the plan from a prior employer’s qualified plan or an IRA 

Investments

You decide which investment options to use and how much to allocate to each. A listing of the plan’s available investment options can be found under the Plan Information section at www.SecurianRetirementCenter.com.

The plan’s optional TargetAge™ feature provides a simple approach to select and maintain a suggested asset allocation based on your age. TargetAge uses a series of allocation portfolios based on age and the investments available in the plan to gradually and automatically shift your entire account’s asset allocation to a more conservative mix of fixed income and equities as you age. With TargetAge, your account is periodically rebalanced. When enrolling in 401(k) on ExponentHR, TargetAge is the default investment allocation. 

You can change how your existing balance and/or future contributions are invested at any time. To make changes, select the “Change my Investments” link on the home page after you log into your account at www.SecurianRetirementCenter.com

You can invest in a pre-mixed model portfolio based on your risk tolerance. A short quiz will help you determine your risk tolerance. The portfolios are selected by the plan sponsor and are made up of several investment options within the plan. Employee Stock Ownership Plan (ESOP)

As an ESOP company, Symbria is able to express our culture through our ownership structure; the ESOP helps us recognize and celebrate the direct impact our employees have on our success. An ESOP is a type of retirement trust that allows employees to have an ownership stake in the company. In Symbria’s case, the company is 100% employee-owned. Each year, the Symbria ESOP allocates shares of company stock into retirement accounts for all eligible employees. Your ESOP retirement savings will grow through these continuing annual allocations as the number of shares you own increases. The value of those shares may increase as well. Participation in the ESOP for eligible employees is automatic. Basic Life and AD&D Insurance 

Symbria offers and pays the cost of Basic Life/AD&D Insurance through VOYA. This helps ease your loved ones ’ financial burden should something happen to you. Your designated beneficiary will receive a benefit if you pass away. In addition, Accidental Death and Dismemberment (AD&D) provides a second benefit to your beneficiary if you pass away or become dismembered as a result of a covered accident. Voluntary Term Life and AD&D Insurance 

Symbria offers Voluntary Term Life/AD&D insurance through VOYA. This coverage allows you to purchase additional coverage at your own financial expense to ease your loved ones’ financial burden if something should happen to you. If you purchase coverage on yourself, you’re also eligible to purchase coverage for your family members. 

An employee’s maximum benefit election cannot exceed 5x their basic annual earnings. A spouse’s maximum election cannot exceed 100% of what the employee takes out on themselves.

Symbria offers Voluntary Accident coverage through Aflac. Accident coverage pays a cash benefit directly to you to help with out-of-pocket expenses when an accident occurs. Accidents are unexpected, so a lump sum payment can help pay medical costs. Here are some, certainly not all, ways that you can trigger payments from your accident policy: 

  • Wellness: Pays a specific benefit amount for going for your FREE annual physical
  • Treatment: Pays a specific benefit amount for emergency-room treatment, X-rays, diagnostic exams, physical therapy and follow-up treatment 
  • Ambulance: Pays a specific benefit amount for ambulance or air-ambulance transportation to a hospital 

  • Hospital confinement: Pays a specific benefit amount for each day the policyholder is confined in a hospital due to injuries sustained in a covered accident and sickness rider 

  • Miscellaneous: Pays a specific benefit amount for concussions, breaks, sprains, burns, dislocations, lacerations, and more

Note, this coverage only applies to accidents that occur off the job.

This open enrollment period is the only opportunity to enroll in benefits until the next annual enrollment period unless you experience a qualifying life event.

What is a qualifying life event? ▼

Certain coverages allow limited changes to your benefit elections during the year. These benefits include the Medical, Dental, Vision, Life, and Disability. For these benefits, you may only make changes to your elections during the year if you have a qualifying life event.

Qualifying life events include:

  • Change in legal marital status, including marriage, divorce, death of a spouse, legal separation, or annulment
  • Change in number of children, including birth, adoption, placement of adoption, or death of a child
  • Change in employment status of employee, spouse, or dependent, including termination or commencement of employment, commencement or return from unpaid leave of absence
  • Change in dependent eligibility status, including attainment of age, student status, or any similar circumstance

The change to your benefit elections must be consistent with the qualifying life event. You have 30 days from the qualifying life event to request your enrollment change.
