What You Need to Know

New Enrollment Process ▼

This is an active enrollment, meaning you must take action to ensure coverage for 2026. Your benefits will not automatically roll over into the new plan year.

There are two ways to elect coverage: 

1. With the Support of a Benefit Counselor

New enrollment process! You have the opportunity complete an enrollment appointment with a professional benefit counselor to elect benefits for 2026. During your individual appointment, you will have the opportunity to ask questions, get more information, and enroll yourself and your dependent(s) in coverage.

2. Enroll online via the Questco Employee Self Service Center

  • If you do not have a username or password, please click “Register” and follow the prompts to register.

  • If you forgot your username or password, please use the “Forgot Username” or “Forgot Password” prompts to reset your password.

  • Please ensure your pop-up blocker has been disabled before continuing to the enrollment portal.

Documents Needed ▼

In preparation for your enrollment appointment, please be ready to provide the following information: 

  • Date of Birth
  • Social Security Number (if applicable)
  • Address & Phone Number
  • Beneficiary Information: name, phone number, DOB, and address (if applicable)
  • Dependent Information: name, phone number, DOB, address, and SSN (if applicable)

SOCIAL SECURITY NUMBERS: Health insurers are required to report certain information about health plan members and their insurance coverage to the IRS each year, including member Social Security numbers. Be sure to have the Social Security number for each dependent you are enrolling in coverage.

Know Your Benefits ▼

KNOW YOUR BENEFITS: Benefits are a big part of your total compensation, so it’s important that you get the maximum value from your benefits plans. Be sure to review all benefits carefully and make the elections that fit your personal or family’s needs.

CHOOSE YOUR BENEFITS CAREFULLY: Your elections will remain in effect until the next open enrollment period, unless you experience a qualifying life event.

How to Enroll

Benefits are more important than ever and we encourage you to spend extra time evaluating your benefit options. As such, we have partnered with EOI Service Company so you can enroll with the guidance of a professional benefit counselor.

It is essential you speak with a benefit counselor to learn more about your benefit options. During your confidential appointment, the benefit counselor will:

  • Answer your questions
  • Provide detailed information
  • Help you enroll in benefits

It’s easy! Just log into the Questco Employee Self Service Center. Select “My Benefits,” then “Benefits Enrollment.” Continue through the prompts until you have submitted your elections and get a confirmation.

Term Life Insurance ▼

Life insurance is a great way to protect your most important assets and help provide the peace of mind your family deserves. This plan pays cash directly to your beneficiary to use however they choose.

EMPLOYEE COVERAGE

  • You can elect up to $100,000 in coverage on a guarantee issue basis.
  • Portability allows you to keep this coverage if you change employers or if benefits are no longer available to you.
  • Rates do not increase for 10 or 20 years
  • At the end of the term period, policy automatically renews at current age, regardless of health.

CHILD(REN) COVERAGE

  • Up to $10,000
  • You can elect to cover your child(ren) if you have employee coverage

SPOUSE COVERAGE

  • Up to $10,000
  • You can elect to cover your spouse if you have employee coverage. Spouse coverage cannot exceed your (the employee) coverage amount.
